摘要
目的:探讨溶栓剂和(或)内皮素受体拮抗剂对于实验犬急性肺血栓栓塞症(PTE)肺动脉压(PAP)的影响。方法:20只草犬随机分为假手术组、模型对照组、溶栓剂尿激酶组(UK组)、内皮素受体拮抗剂组(BQ123组)和尿激酶联合BQ123组(联合组)共5组,分别给予假手术操作、注入自体血栓、注入自体血栓加UK、注入自体血栓加BQ123以及注入自体血栓加UK和BQ123。结果:接受UK和(或)BQ123组PAP升幅(ΔPAP)明显降低。在造模后2.5h,模型组PAP升高(7.70±1.72)mmHg(1mmHg=0.133kPa),而UK组仅升高(5.56±1.26)mmHg,BQ123组升高(3.83±1.60)mmHg,联合组则下降(1.70±0.72)mmHg,P<0.001;与联合组相比,UK组、BQ123组PAP水平均有显著性差异。结论:尿激酶和BQ123均能显著降低PAP,两者合用效果更明显,提示溶栓剂和内皮素受体拮抗剂联合治疗在急性PTE的治疗中具有重要意义。
Objective To investigate the effect of thrombolytic agent and (or) endothelin-1 (ET-1) receptor antagonist on pulmonary arterial pressure(PAP) in experimental pulmonary thromboembolism (PTE). Methods Twenty mongrel dogs were randomly divided into five groups: sham operation group, model group, thrombolytic agent group (UK group), ET-1 receptor antagonist group (BQ123 group) and thrombolytie agent plus ET-1 receptor antagonist group (combination group); the abovementioned groups were received sham operation, injecting autologous blood clots, blood clots plus urokinase, blood clots plus BQ123,and blood clots plus UK and BQ123, respectively. Results Mean PAP (mPAP) decreased remarkably in groups that received UK and(or) BQ123. At 2.5 h after the establishment of model, mPAP increased by(7.70±1.72) mm Hg(1 mm Hg=0.133 kPa) in model group,by (5.56±1.26) mm Hg in UK group, and by (3.83±1.60) mm Hg in BQ123 group, but decreased by (1.70±0.72) mm Hg in combination group (P〈0.001). The difference of mPAP between combination group and UK group or BQ123 group was statistically significant. Conclusions Thrombolytic agent and ET-1 receptor antagonist both may significantly reduce PAP in experimental acute PTE, and their combined effect is more significant. These results indicate that combined use of thrombolytic agent and ET-1 receptor antagonist will be useful in the treatment of acute PTE.
