超声造影对前列腺癌诊断的临床应用研究 被引量:8

Clinical application of contrast enhanced ultrasonography in the diagnosis of prostate cancer

摘要 目的:探讨超声造影(CEUS)在前列腺癌诊断中的临床应用价值。方法:应用超声造影剂声诺维(SonoVue)对经直肠穿刺病理证实的24例前列腺癌患者共32个外腺癌结节进行CEUS检查,观察结节的造影增强表现,并且绘制时间-强度曲线(TIC),定量分析造影剂到达时间(AT)、强度达峰时间(TTP)及峰值强度(PI)参数。结果:32个结节中,与外腺正常组织相比,造影呈现高增强的有27例,等增强和低增强各2例,无增强1例。31例增强结节的AT和TTP分别为(16.9±6.1)s和(29.1±9.7)s,低于正常外腺组织(20.2±6.6)s和(35.0±7.9)s(P<0.05),PI为(9.6±4.0)dB,高于正常外腺组织(7.5±3.8)dB(P<0.05)。结论:CEUS能较为客观地评价前列腺癌病灶内的血流供应情况,可以引导进行病灶的活检,TIC能对病灶进行定量参数分析,在临床上有助于前列腺癌的早期诊断。 Objectlve:To evaluate the clinical appilieation of contrast-enhanced ultrasound (CEUS) in the diagnosis of prostate cancer. Methods:Twenty-four prostate cancer patients confirmed by transrectal biospy with 32 nodules in the outer gland underwent CEUS with a bolus injection of SonoVue. The enhancement appearance of all the nodules were overviewed and the time-intensity curves (TIC) were drawn to record and analyze the parameters of contrast agent arrival time (AT),time to peak intensity (TTP) and peak intensity (PI). Results :Twenty-seven, 2,2 and 1 of 32 nodules showed increased, equal, decreased and no enhancement, respec- tivery', compared with surrounding outer gland tissue. The AT and TTP of 31 enhancement nodules( 16.9 ± 6.1 )s and(29.1±9.7)s, respectively were lower than that of the normal outer gland tissues(20.2± 6.6)s and(35.0 ±7.9)s, respectively (P 〈 0.05) ,and the PI(9.6 ± 4.0)dB higher than that of the nomlal(7.5±3.8)dB (P 〈 0.05). Conclusion:CEUS could reveal the presence of vasculature within the lesions of prostate cancer objectively and guide the prostate biopsy and TIC could be used in the quantity analysis of the lesions. CEUS could be helpful in the early diagnosis of the prostate cancer.
