舒芬太尼复合小剂量罗哌卡因腰麻在日间手术的应用 被引量:9

摘要 目的观察舒芬太尼复合小剂量罗哌卡因的腰麻效果,探讨其在日间手术应用的有效性和安全性。方法90例患者,随机分为三组,每组30例。A组:罗哌卡因7.5mg(对照组);B组:罗哌卡因7.5mg加舒芬太尼5μg;C组:罗哌卡因2.5mg加舒芬太尼5μg,三组均以10%葡萄糖溶液加至2ml。监测记录患者循环功能、感觉阻滞程度、运动阻滞程度、留院时间及镇痛维持时间。结果用药后A、B两组MAP明显下降、HR减慢(P<0.05或P<0.01),但C组变化不明显;C组与A、B两组比较均有统计学意义(P<0.05或P<0.01)。各组用药后VAS显著降低(P<0.01),组间比较差异无统计学意义;A、B两组用药后Lovett等级评分显著降低(P<0.01),C组则无明显变化,但显著高于A、B两组(P<0.01)。A、B两组患者的留院时间、镇痛维持时间均显著长于C组(P<0.01)。结论舒芬太尼复合小剂量罗哌卡因腰麻对血流动力学的影响小、感觉阻滞完善、运动阻滞弱、留观时间短,用于日间手术的麻醉安全、有效。
