超声斑点追踪成像技术对糖尿病患者左心室收缩功能早期变化的研究 被引量:10

摘要 目的:应用超声斑点追踪成像技术评价2型糖尿病患者左心室短轴应变及左心室扭转运动的变化规律。方法:单纯糖尿病患者41例,糖尿病合并左心室肥厚患者25例,正常对照者37例。获取胸骨旁短轴二尖瓣水平、乳头肌水平、心尖水平二维图像,测量左心室径向和圆周收缩期局部及整体峰值应变、左心室扭转角度。结果:与对照组比较,两组糖尿病患者径向各应变参数值降低,但差异无统计学意义(P>0.05);两组糖尿病患者部分心肌节段、不同水平平均峰值应变及左心室整体圆周应变较对照组减低(P<0.05),糖尿病合并左心室肥厚组低于单纯糖尿病组(P<0.05)。两组糖尿病患者左心室扭转角度较对照组增大(P<0.05),糖尿病合并左心室肥厚组高于单纯糖尿病组(P<0.05)。结论:超声斑点追踪成像能够较早识别糖尿病患者左心室收缩功能异常。 Objective To investigate the applied value of assessing the early cnange of left ventricular twist and short-axis strain patients with type 2 diabetes mellitus by ultrasound speckle tracking imaging. Methods Myocardial imaging of 41 with diabetes mellitus only (DM group), 25 with both diabetes and left ventricular hypertrophy (DH group), and 37 normal controls was collected. Left ventricular peak systolic global and segmental radial strain and circumferential strain, left ventricular systolic global and segmental rotation, and left ventricular peak systolic global twist were assessed respectively. Results Left ventricular peak systolic radial strain parameters were not significant difference among three groups (P〉0.05). Compared with control group, left ventricular peak systolic circumferential strain of part segments ,average peak strain at different levels and global peak strain in all patients were significantly reduced (P〈0.05). The parameters in DH group were significantly lower than those in DM group. Left ventricular twist were significantly higher in DM group and DH group than those in control group (P〈0.05), and the parameter in DH group was greater than that in DM group (P〈0.05). Conclusion Ultrasound speckle tracking imaging could be used to evaluate the early change of left ventricular systolic function in diabetic patients.
