2型糖尿病肾病患者血清超敏C-反应蛋白、脂联素测定的临床意义 被引量:2

Clinical significance of serum high sensitive C-reactive protein and adiponectin level in the diagnosis of type 2 diabetic patients with nephropathy

摘要 目的探讨超敏C-反应蛋白(hs-CRP)及脂联素在2型糖尿病肾病诊断中的水平变化、相互关系及临床应用价值。方法选择2型糖尿病住院患者136例作为观察对象,并根据尿白蛋白排泄量分为3组:极微量白蛋白尿(n=42),微量白蛋白尿组(n=44),大量白蛋白尿组(n=50);同时选择健康体检者50例作为正常对照组。随后分别用胶乳增强免疫比浊法测定hs-CRP、ELISA法测定脂联素,同时测定空腹血糖(FBG)、血肌酐(Scr)、血脂水平、尿白蛋白(UAlb),比较各组间的差异。结果2型糖尿病肾病组hs-CRP水平随尿白蛋白排泄量和Scr水平的增加而升高,而hs-CRP与血清脂联素水平呈负相关。结论hs-CRP及脂联素联合监测对于2型糖尿病早期肾损伤诊断有重要参考价值。 Objective To investigate relationship between serum adiponectin, high sensitive C-reactive protein(hs-CRP)and levels of urinary albumin(UAlb)excretion amount in type 2 diabetic patients with different stages of nephropathy. Methods A total of 136 type 2 diabetic patients with nephropathy and 50 normal healthy controls were enrolled in this study. According to UAIb excretion amount, these patients were divided into three groups:trace albuminura group( 〈 30 mg/d, n = 32), microalbuminura group(30- 300 mg/d, n = 44) and macroalbuminura group ( 〉 300 mg/d, n = 50). The levels of serum adiponectin, hs-CRP, blood-fat, Scr, fasting blood glucose(FBG) ,Ualb excretion amount were measured and compared. Results The level of serum adiponectin actually increased following the increase of UAlb excretion amount and Scr. Correlative analysis revealed a negative correlation between hs-CRP and serum adiponectin. Conclusion The combination of serum adiponectin and hs-CRP may play an important role in early diagnosis of diabetic nephropathy.
