护士分层次管理在呼吸科的应用 被引量:21

摘要 目的探讨护士分层次管理在呼吸科的作用,寻找适合病房工作需要和护士发展的最佳管理模式。方法2006年3月—2007年2月采取传统的整体护理管理方法,科内护士轮流担任责任组长及其他各班次。2007年3月—2008年2月期间采取分层次管理的方法,即依据职称、能力、学历等综合能力及呼吸科工作特点将护士分成4个层次:责任组长、执行护士、辅助护士、秘书。各层次人员严格制定任职条件、工作职责、奖金待遇,根据各自工作重点有针对性地进行考核。比较分层次管理前后护士、患者满意度情况及护理质量考核成绩。结果(1)分层次管理实施前后15名护士对护士之间的团结性、积极性、工作效率等9方面满意度评分比较,差异具有统计学意义(P<0.05或P<0.01),实施分层次管理后护士满意度评分明显提高。(2)实施分层次管理后患者在解决问题的及时性和疾病需求的满足等9方面满意度明显提高(P<0.05或P<0.01)。(3)实施分层次管理前后基础护理、专科护理、病历书写质量考核成绩比较,差异具有统计学意义(P<0.01),实施分层次管理后,护理工作质量考核成绩高于实施前。结论护士的分层次管理提高了护理工作质量和效率;人尽其才,调动了护士工作积极性和团结性;加快了年轻护士的成长;确保了护理工作的安全性和及时性,特别是在危、重、急患者多的科室值得推广。 Objective To explore the the application of tiered nursed management in respiratory department and thus find out the best management mode answering the needs of ward work and nurse development. Methods Between Mar. 2006 and Feb. 2007, the traditional integrated nursing management was adopted, with the intra-department nurses taking turns acting as the responsible head and working various shifts. Between Mar. 2007 and Feb. 2008, the tiering system was put into practice, in which the nurses were, according to their professional titles, abilityies, academic qualifications, etc., divided into 4 tiers, namely, responsible heads, executive nurses, assistant nurses and secretaries. Qualifying requirements, job duties, bonuses and other treatments for the various tiers were formulated, and checking and examination were conducted by taking into accouont the key work points with the tiers. Then a comparison was made of the nurses' and patients' satisfaction and the nurses' scores in the examniation before and after the introduction of tiering system. Results (1) The difference in 15 nurses' assessment of their work solidarity, initiative and efficiency before and after the adopting of the tiered system was statsitically significant (P〈0.05 or P〈0.01), with the nurses' satisfaction increasing markedly after the tiered management. (2) The patients' satisfaction in 9 aspects such as the betiming in solving their problems and meeting their needs related to their illnesses also saw an significant increase (P〈0.05 or P〈0.01). (3) The differences in the scores of examinations covering the basic nursing, specialized nursing, medical record writing before and after the use of the tiering system was significant (P〈0.01), with those after the practice of the system better than before. Conclusion Tiered nursed management improves nursing quality and efficiency, brings nurses' intiative into full play, strengthens their solidarity, faciliates their development and ensures nursing safety and immediacy. Therefore, it is worth being popularized, especialy in departments with lots of patients whose diseases are serious and emergent.
