摘要
目的:探讨血浆内皮素1(ET1)与新生儿窒息及窒息后脑损伤的关系。方法:用放射免疫法测定20例正常足月新生儿和53例足月窒息新生儿血浆ET1浓度,并观察其动态变化(生后24小时内、第3日和第7日)。结果:患儿生后3日内重度窒息组血浆ET1浓度显著高于对照组和轻度组(P均<0.01),且轻度组高于对照组(P<0.01);第7日窒息组ET1浓度逐渐下降,轻度组与对照组无显著差异,但重度组仍高于对照组和轻度组(P均<0.01)。生后24小时内ET1浓度最高的5例患儿4例死亡,第7日ET1浓度高于正常值上限的14例中5例3个月后头颅CT显示有脑萎缩。结论:ET1水平与新生儿窒息程度呈正相关;ET1在24小时内显著升高及第7日持续不降者预后不良。
Objective:To investigate changes in plasma endothelin1(ET1) levels and their relationship to the development of secondary brain damage in neonates with asphyxia.Methods:In 53 asphyxia infants (28 cases with mild degree and 25 cases with severe degree) and 20 normal infants,plasma ET1 concentrations were measured by radioimmunoassay within 24 hours,on days 3 and 7 postnatal.Results:ET1 concentrations in neonates with severe asphyxia were much higher than those with mild asphyxia and normal controls (both P<001) within 3 days postnatal,and neonates with mild asphyxia had high ET1 values compared to normal controls (P<001).On postnatal day 7,plasma ET1 concentrations gradually decreased,and it was still significantly high in neonates with severe asphyxia compared to those with mild asphyxia as well as normal infants (both P<001).Also,4 out of 5 cases with the highest ET1 concentrations within 24 hours postnatal died,and 5 out of 14 cases with ET1 concentrations over the normal range on postnatal day 7 were diagnosed as brain atrophy by CT scan 3 months later.Conclusions:The results suggest that ET1 levels are associated with the degree of asphyxia and secondary brain damage.Marked elevation in plasma ET1 concentrations within 24 hours postnatal and persistent high ET1 values may carry out a poor outcome in neonates with asphyxia.
