实时超声造影在胆囊癌诊断中的应用研究 被引量:5

摘要 目的探讨低机械指数实时超声造影在胆囊癌诊断中的应用价值。方法回顾分析32例经手术病理证实为胆囊癌的超声造影表现,观察病灶开始增强时间、达峰时间及持续时间,并比较病灶增强形态。结果超声造影诊断胆囊癌32例,经手术病理证实胆囊癌30例,良性病例2例,诊断符合率为93.8%。胆囊癌超声造影表现为病灶与胆囊壁同时显影,肝动脉期以高增强为主,肝门静脉期以低增强为主,肝实质期低增强或无增强。结论胆囊癌超声造影回声增强具有特征性,有助于提高胆囊恶性病变的检出,超声造影对胆囊疾病的诊断和鉴别诊断有较好的应用前景。 Objective To explore the application value of low - mechanical index enhanced grey scale ultrasonography in diagnosis of gallbladder cancer. Methods Thirty - two patients with gallbladder cancer confirmed by surgery and pathology were enrolled in this study. Their appearances of contrast enhanced ultrasound were analyzed retrospectively. The time of initial enhancement, peak enhancement, and initial decrease were observed, and enhancement appearances of lesion were compared. Results Thirty - two cases of gallbladder cancer were diagnosed by contrast enhanced tdtrasound, 30 cases of them were confirmed by surgery and pathology, 2 cases were benign, the diagnose accordance rate was 93.8 %. The common characteristics of gallbladder cancer were: focus and gallbladder wall were enhanced at the same time, hepatic artery phase was mainly highly enhanced, hepatic portal vein phase was mainly lowly enhanced, liver parenchyma phase was lowly or not enhanced. Conclusion Enhanced grey scale ultrasonography has characteristics in diagnosis of gallbladder cancer, which can improve the diagnose accordance rate of gallbladder cancer, it has good prospect in differential diagnosis of this disease.
