精神分裂症与抑郁症、神经症听觉P300的比较 被引量:17

A comparative study of P 300 ERPs among schizophrenia and depression and neurosis

摘要 目的探讨精神分裂症、抑郁症和神经症听觉事件相关电位P300的特点。方法对20例精神分裂症、18例抑郁症和神经症患者与20例正常人的P300进行对照研究。记录Cz电极位置P300的N2、P3成分,并进一步观察经药物治疗的精神分裂症组和抑郁症组P3成分治疗前后的变化。结果精神分裂症组与正常对照组比较P3潜伏期均值延长58毫秒,波幅均值下降8.4μV,但与抑郁症组相比未发现这些成分差异有显著性。治疗后精神分裂症组患者临床症状明显好转,但P3成分却未发现明显变化。抑郁症组患者经抗抑郁药物治疗后,伴随着临床症状的消失或好转及汉密尔顿抑郁量表得分下降,P3潜伏期缩短,波幅增高。结论提示作为反映精神分裂症、抑郁症认知功能障碍的客观生理指标,P300有可能作为精神分裂症临床诊断的一个生理学特征性标志。 Objective The purpose of this study was to explore the characters of P 300 auditory ERP in schizophrenia, depression and neurosis. Methods P 300 was measured in 20 patients with schizophrenia, 18 with depression and neurosis and 20 normal controls. The N 2, P 3 components of P 300 were recorded in Cz electrode position. And the relationship between the changes of P 300 and therapeutic effect on schizophrenia an depression was observation. Results The latency of P 3 prolonged for 58 ms and amplitude of P 3 reduced for 8.4 μV in schizophrenia compared with control group, but no significant difference was noted between schizophrenia and depression. Schizophrenia patients were improved after treatment, but no change was noted in P 3. Depression patients showed an increased P 3 amplitude and a shortened P 3 latency with the improvement of clinical symptoms after antidepressant treatment. Conclusion auditory P 300 ERP could served as a physiological index in the reflection of the cognitive disorder in schizophrenia and depression.
