无症状性脑梗死患者高敏C-反应蛋白、同型半胱氨酸、纤维蛋白原和血脂成分分析 被引量:18

Analysis of high-sensitivity C-reactive protein,homocysteine,fibrinogen and serum lipid components in patients with silent cerebral infarction

摘要 目的:对无症状性脑梗死患者的高敏C-反应蛋白、同型半胱氨酸、纤维蛋白原及血脂不同成分进行分析,探讨上述指标与无症状性脑梗死的关系。方法:高敏C-反应蛋白水平用免疫散射比浊法检测,同型半胱氨酸浓度用酶联免疫吸附法测定,纤维蛋白原用Thrombotimer4自动血凝仪测定,用Olimpus-AU5400全自动生化分析仪进行血脂相关成分的分析,共检测无症状性脑梗死患者216例,对照组210例。结果:无症状性脑梗死组高敏C-反应蛋白、同型半胱氨酸、纤维蛋白原和脂蛋白(α)的水平均较对照组明显升高(均P<0.01),而载脂蛋白A1水平则显著低于对照组(P<0.01);三酰甘油、总胆固醇、高密度脂蛋白、低密度脂蛋白和载脂蛋白B2组间比较差异无统计学意义(均P>0.05)。结论:高敏C-反应蛋白、同型半胱氨酸、纤维蛋白原和脂蛋白(α)升高及载脂蛋白A1降低均是无症状性脑梗死患者的高危危险因素。 Objective To analyze high-sensitivity C-reactive protein, homocysteine, fibrinogen, and different components of serum lipid in patients with silent cerebral infarction and to explore the relationships between them and silent cerebral infarction. Methods The level of serum high-sensitivity C-reactive protein was measured with immunodiffusion turbidimetry. The serum homocysteine level was detected with ELISA. The plasma fibrinogen level was measured with thrombotimer 4 automatic blood coagulation instrument. The concentrations of serum lipid components were analyzed on the Olympus-AU5400 automatic biochemistry analyzer. A total of 216 silent cerebral infarction patients and 210 cases of control group were measured. Results The high-sensitivity C-reactive protein, homocysteine, fibrinogen and lipoprotein (a) levels in silent cerebral infarction group were significantly higher than those in control group, ( 18. 72 ± 5. 20) mg/L vs ( 11. 33 ± 3. 61 ) mg/L, (22.21±6.21) umol/L vs (15.32±2. 82) umol/L, (5.23±2. 31) g/L vs (2. 84±1. 46) g/L and (279.36±73.25)mg/L vs (214.24±47.68)mg/L, respectively (all P〈0.01), but the concentration of ApoA1 in silent cerebral infarction group was markedly lower than that in control group, (1.33± 0.28)g/L vs(1. 43±0.32)g/L, (P〈0.01). The concentrations of TG, TC, HDL, LDL and ApoB had no statistical significance between silent cerebral infarction group and control group (all P 〉0.05). Conclusion Increased high-sensitivity C-reactive protein, homocysteine, fibrinogen, lipoprotein (a) and decreased ApoB1 are all high-risk factors in patients with silent cerebral infarction.
