摘要
目的探讨丝裂霉素C(MMC)对兔角膜准分子激光切削术(PRK)后角膜愈合、角膜上皮下混浊及转化生长因子β2(TGF-β2)表达的影响。方法24只健康新西兰白兔(48眼)分两组行PRK手术,术后将浸有0.02%MMC的滤纸片放置在右眼角膜中央切削区(MMC组),将浸有平衡盐水(BSS)的滤纸片放置在左眼角膜中央切削区(BSS组)2 min后用150 ml BSS冲洗。同时设立正常对照组。观察角膜上皮愈合情况、角膜浅基质层角膜细胞数量和形态变化及角膜上皮下混浊;免疫组化检测正常对照组及手术组(MMC组和BSS组)术后1 d、1周、4周和12周TGF-β2表达情况并作分析。结果MMC组角膜上皮愈合时间为(3.0±0.69)d,对照BSS组为(3.0±0.71)d,两组差异无统计学意义(P>0.05);术后MMC组浅基质层内角膜细胞明显减少,两组差异有统计学意义(P<0.05);BSS组角膜上皮下混浊明显重于MMC组,两组差异有统计学意义(P<0.05);术后各时段BSS组TGF-β2表达多于MMC组。结论MMC不会延长角膜上皮的修复时间,并减轻术后角膜上皮下的混浊;MMC减轻术后角膜上皮下混浊形成可能部分与其调节TGF-β2的表达有关。
Objective To investigate the effect of mitomycin C corneal wound healing,haze and expression of transforming growth factor-β2 after photorefractive keratectomy (PRK ) in rabbits. Methods Twenty-four New Zealand rabbits divided randomly into MMC group and BSS group underwent bilateral 193 nm excimer laser photorefractive keratectomy to correct -6. 0 diopter of myopia. After surgery a filter soaked with 0. 02% MMC was placed on the central ablation stromal bed in the right eye and BSS in the left eye for 2 minutes then irrigated with 150 ml BSS. The process of epithelialization was observed . Corneal haze was assessed biomicroscopically using Fantes scale. The number of superficial stromal keratocytes was counted and morphologic changes were observed. Expression of TGF-β2 was detected and analyzed by immunocytochemical method at 1 d, 1 week,4 week and 12 week after surgery. Results Mean epithelialization time was ( 3. 0 ± 0. 691 days in the MMC eyes and 3. 0 ± 0. 71 days in the control BSS eyes ( not statistically significant ,P 〉 0. 05 ). A remarkable reduction of keratocytes in the superficial stroma after surgery was observed in MMC group(P 〈0. 05). Corneal haze was more severe in BSS group than in MMC group. Expression of TGF-β2 in corneas had increased more in BSS group than in MMC group at all time. Conclusion MMC did not delay epithelialization and could lessen the development of haze after photorefractive keratectomy. The effect of MMC on inhibiting haze induced by PRK maybe mediated through the suppression of TGF-β2.
