摘要
目的探讨脑梗死急性期行高压氧治疗对患者1年内神经功能恢复的影响。方法急性脑梗死患者192例分为高压氧组和非高压氧组,分别于治疗前、治疗20d,3个月、6个月、12个月时进行神经功能缺损评分对比。根据治疗前神经功能缺损评分,每组患者再被分为轻、中、重3个亚组进行分析。结果(1)神经功能缺损轻型组:治疗后各评分时间高压氧组均较非高压氧组有显著改善(P〈0.01或P〈0.05)。(2)神经功能缺损中型组:高压氧20d组、12个月组较同期非高压氧组有明显改善,差异有统计学意义(P〈0.05)。(3)神经功能缺损重型组:高压氧6个月组、12个月组较同期非高压氧组有明显改善,差异有统计学意义(P〈0.05)。(4)高压氧组1年死亡率较非高压氧组有明显降低,差异有统计学意义(P〈0.01)。高压氧组1年复发率较非高压氧组有明显降低,差异有统计学意义(P〈0.05)。结论脑梗死患者急性期行高压氧治疗不仅可以改善患者急性期神经缺损,而且在1年内仍可使患者受益,改善神经功能,降低死亡率及复发率。
Objective To explore curative effects of hyperbaric oxygen (HBO) combined therapy for the patients with one-year acute cerebral infarction. Methods One hundred and ninty-two patients with acute cerebral infarction were divided randomly into HBO group and control group. The neurological impairment was scored before and after 20 days, 3 months, 6 months and 12 months of HBO therapy,respectively. According to the severity of neurological impairment before therapy, each group was divided into three subgroups. Results ( 1 ) Slight neurological impairment group : neurological impairment scores of HBO group were significantly lower than those of simultaneous control group( P 〈 0.01 or P 〈 0.05 ). (2) Moderate neurological impairment group : neurological impairment scores of HBO group after 20 days and 12 months were significantly lower than those of simultaneous control group ( P 〈 0.05 ). ( 3 ) Sever neurological impairment group: neurological impairment scores of the HBO group after 6 and 12 months were significantly lower than those of simultaneous control group (P 〈 0. 05 ). (4) Both one-year mortality and relapse rate were significantly lower in the HBO group than those in control group ( P 〈 0.01 or P 〈 0.05 ). Conclusions During the acute stage, HBO therapy can improve neurological function recovery and benifit the patients after one-year acute of cerebral infarction. At the same time, HBO therapy can cut clown the rate of mortality and relapse.
