摘要
目的:探讨LASEK术中使用乙醇软化角膜上皮的理想时间和影响上皮瓣制作的原因。方法:总结2007-01/2008-03在我院行LASEK的581例(1146眼)使用200mL/L乙醇采取不同时间软化角膜制作上皮瓣,其中15s者67眼,18s者230眼,20s者271眼,22s者70眼,25s者235眼,28s者273眼。术前配戴角膜接触镜0.5a以上者424眼(37.00%)。结果:上皮瓣制作成功1019眼(88.92%),各时间组的成功率分别是:15s者58.21%,18s者85.65%,20s者87.82%,22s者91.43%,25s者94.47%,28s者94.87%。术前配戴角膜接触镜的424眼的成功率分别是15s者33.33%,18s者71.43%,20s者75.00%,22s者87.50%,25s者87.21%,28s者88.10%。无角膜接触镜配戴史722眼的成功率分别是:15s者72.09%,18s者96.21%,20s者95.32%,22s者94.74%,25s者98.66%,28s者97.88%。结论:LASEK术中采用200mL/L乙醇浸泡角膜的合理时间在术前无角膜接触镜配戴史的是18s,配戴角膜接触镜0.5a以上的应延长到25s,长期配戴角膜接触镜是影响上皮瓣制作的主要原因。
AIM： To approach the ideal time of softening corneal epithelium with alcohol and the reasons contributing to making epithelium flap in LASEK. METHODS： We summarized the cases who accepted LASEK in our hospital from January 2007 to March 2008, and in the operation 200mL/L alcohol was adopted to soften corneal epithelium with different time. Of all 581 cases （1 146 eyes）, 67 eyes was for 15 seconds, 230 eyes for 18 seconds, 271 eyes for 20 seconds, 70 eyes for 22 seconds, 235 eyes for 25 seconds, and 273 eyes for 28 seconds. 424 eyes （37.00%） wore contact lens with at least half a year. RESULTS： Totally 1 019 eyes （88. 92%） were with successful corneal epithelium flap. The achievement ratios with different time were 58. 21% in 15-second group, 85.65% in 18-second groupd, 87.82% in 20-second group, 91.43% in 22-second group, 94.47% in 25-second group, and 94.87% in 28-second group respectively. As to the 424 eyes with a history of contact lens, the achievement ratios were 33. 33%, 71. 43%, 75. 00%, 87.50%, 87.21% and 88.10% respectively. The achieve-ment ratios of 722 eyes without contact lens were 72.09%, 96.21%, 95. 32% ,94.74% ,98.66% and 97.88% respectively. CONCLUSION： The reasonable time of making corneal epithelium with 200mL/L alcohol in LASEK is 18 seconds for the cases without a history of contact lens, and we should lasting 25 seconds for the ones who have been wearing contact lens for half a year or more. The main cause influencing the epithelium flap making is the history of wearing contact lens for a long time.
