摘要
目的测量并比较实施护士分层级管理前后基础护理终末质量。方法选择上海交通大学附属第一人民医院松江分院2007年7月至2008年6月在职的综合性重症监护病房(GICU)的全体护士,根据卫生部1985年试行的专业技术职务聘任制对专业技术职务的岗位设置和配备比例规定,以及《上海市卫生局关于护士分层分级管理的初步研究报告》的相关规定,实施护士分层级管理。比较实施护士分层级管理前、后6个月,GICU病区基础护理终末质量及护士满意度。结果GICU病区患者对护理工作的满意度差别有统计学意义(P=0.001);健康教育覆盖率差别有统计学意义(P=0.000);医院感染发生率差别无统计学意义(P>0.05);预报褥疮发生率差别无统计学意义(P>0.05);护理差错、护患纠纷差别无统计学意义,发生率均为0。结论二级综合性医院ICU实施护士分层级管理有利于提高出院患者满意度、健康教育覆盖率,对其他基础护理终末质量指标的影响则有待进一步证实。
Objective To measure and compare the basic nursing quality and the patients' satisfaction before and after the nurse grading administration conducted. Methods Nurses working in ICU of the Songjiang Branch Hospital of The Shanghai First People's hospital afflicted to Shanghai Jiao Tong University From July 2007 to June 2008 were chosen. They were classified according to the national regulation of the professional classification established in 1985 and the nurse grading system of Shanghai Medical Bureau. The nrsing quality and patients' satisfaction were compared before and after practicing the nursing grading administration for six months in ICU. Results There was significant differenee between the patients satisfaction before and after practicing the nursing grading administration(P=0.001). There was significant difference between the cover ratios of the patient education before and after practicing the nursing grading administration. But there were no significant difference among the hospital infection ratios, the predicted bedsore occurrence ratios,the nursing error and dissension before and after practicing the nursing grading administration (P〉0.05). Conclusions The practice of the nursing grading administration can promote the patients' satisfaction and the cover ratio of the patient education in ICU in second-class comprehensive hospital. Much more studies should be done to prove influences on other items of the ultimate basic nursing quality.
