摘要
目的:了解基层医疗机构医院感染预防与控制现状。方法:从河南省艾滋病医疗救治重点县中随机抽取一县,对该县的31个村卫生室、11个乡卫生院进行"消毒灭菌、医院感染管理状况"问卷调查。随机抽取该县374名医务人员进行"消毒灭菌、医院感染知识"问卷调查。同时采集标本进行检测,村卫生室采集部位主要包括治疗室治疗台及治疗车台面、皮肤消毒液碘伏、酒精及消毒液容器、敷料缸等。乡卫生院采集部位主要包括治疗室、换药室、手术室、人流室、产房、皮肤消毒液、无菌包、手提式压力蒸气灭菌器等。根据河南省卫生厅医政处制订的综合考评标准,对调查村、乡从医院感染管理、教育培训、医务人员防护、重点部门管理、医疗废物管理5个方面进行综合考评。结果:将"消毒灭菌、医院感染知识"问卷调查表项目归类合并为4个领域,即消毒灭菌、医院感染预防与控制重视程度、个人防护、消毒灭菌操作和医疗废物处理,该县4个领域的得分分别为(53.25±2.81),(47.72±2.53),(41.54±1.95)和(20.19±1.08),4个领域得分差异有统计学意义(F=16.259,P<0.05)。对"消毒灭菌、医院感染管理状况"调查表各项目进行评分,满分50分,村卫生室得分(21.00±1.02)分,乡卫生院得分(37.00±1.35)分,2者比较,差异有统计学意义(t=44.915,P<0.05)。村卫生室碘伏、酒精消毒合格率为100%。乡卫生院皮肤消毒液(碘伏、酒精)、无菌包消毒合格率达到100%。其余部位采样检测结果不理想。村卫生室综合考评得分率70%～90%,乡卫生院得分率50%～60%。结论:重视基层医院感染工作,对基层医疗机构医务人员进行医院感染知识的教育培训至关重要,应成为医院感染管理的重要内容之一。
Aim：To study the basic medical staff how to prevent and control nosocomial infection. Methods：The data were got through a cross-sectional study. Results ： The questionnaire results of the 374 medical staff showed that the basic medical staff paid less attention to prevent and control disinfection, isolation and nosocomial infection, that it should be more strengthened for protecting individual ,operating correctly disinfection and isolation and handling medical wastes. The results of the laboratory were not well but the disinfector of the skins and the package of asepsis. The total results found that the score rate was between 70% and 90% in the 31 villages and between 50% and 60% in the 11 towns. Conclusion： It was more important that the basic medical staff paid more attention to prevent and control disinfection, isolation and nosocomial infection, and went on studying about the knowledge of disinfection,isolation and nosocomial infection , which should be one of important contents in managing nosocomial infection.
