摘要
目的:探讨微球囊压迫(PMC)治疗老年(70岁以上)原发性三叉神经痛时的三叉神经抑制反应。方法:回顾性分析从2001年1月至2007年12月采用改良Mullan等人的PMC方法治疗3053例原发性三叉神经痛患者中804例老年患者的临床资料。结果:在对804例老年患者行833次手术中观察到三叉神经抑制反应766次(92%),主要表现为心率降低(平均心率降低25次/分),其中16例出现一过性心脏窦性停搏;手术早期60%患者出现明显的血压下降,操作后逐步恢复到或略高于基础水平。结论:三叉神经抑制反应是微球囊压迫半月节手术过程中的一种常见的心脏抑制反应现象。尤其对老年患者更应充分注意,手术者和麻醉师必须充分意识到这种致命性并发症的发生并做好相应的处理准备。
Objective： To discuss trigeminal depressor response occurred during percutaneous microcompression for aged trigeminal neuralgia patients. Method ： To review the results of 804 aged patients （ age 〉 70 yr） out of 3053 trigeminal neuralgia patients who underwent percutaneous microeompression procedures in Liaoning provincial hospital between Jan 2001 and Dec 2007. Result： 766 trigeminal depressor responses were observed in the 833 procedures performed on this 804 patients with main feature of abrupt drops in heart rate and asystole occurred in 16 cases. Conclusion： Trigeminal depressor response is very common during the procedures of percutaneous microcompression for trigeminal neuralgia. Surgeon and anaesthesist must always be aware of the potential for the trigeminal depressor response and both be prepared surgically and pharmacologically to treat the possible life-threatening consequences intraoperatively.
