老年人无髓后牙粘结银汞桩核的应用评价 被引量:3

Assessment of bonded amalgam cores in preserving elderly patients' endodontically treated posterior teeth

摘要 目的观察粘结银汞桩核在老年人无髓后牙保存修复中的应用效果。方法选择75颗老年人无髓后牙,经根管治疗及龈上洁治术,1~2周后预备剩余牙冠以及根管,酸蚀,涂布树脂粘结剂,充填形成银汞合金桩核。24h后牙体预备、印模、灌模,制作烤瓷熔附金属全冠,粘固后拍摄根尖片。6个月、1年后复查,内容包括临床检查及拍摄根尖片。并将龈沟出血指数与对侧同名牙做配对比较。结果1年内修复体保存率及临床可接受率为100.0%,全冠及桩核无松动,边缘密合,可发挥正常的咀嚼功能,根尖片未显示根折及桩折,银汞合金桩与根管壁、根端牙胶紧密接触。牙龈无明显红肿,其龈沟出血指数与对照牙之间差异无统计学意义(P>0.05)。结论粘结银汞桩核用于老年人无髓后牙保存修复可取得良好的效果。 Objective This in vivo study was undertaken to evaluate the effect of bonded amalgam cores being used in preserving elderly patients' endodontically treated posterior teeth.Methods Seventy-five endodontically treated posterior teeth were selected from 62 elderly patients in this study.All the teeth had been endodontically treated and performed with ultrasonic scaling.After 1~2 weeks,the crowns and root canals were prepared,then the composite bonding agent and mixed amalgam was filled into the canals and pulp chambers tightly.Tooth preparation was operated and the impressions were taken after 24 h,master casts were gained and porcelain-fused-to-metal crowns were fabricated in the lab.The crowns were cemented with glass ionomer cement.X-ray radiographs were taken after cementing.The further consultations were offered to these teeth with examination and radiographs after 6 months and 1 year.The difference of sulcus bleeding index among these endodontically treated posterior teeth and the same teeth in the opposite sides was pairedly compared.Results In these cases,100.0% of crowns and amalgam cores were doing function well with no exhibiting mobility after 6 months and 1 year.No abnormal image displayed on the X-ray radiographs.There was no significant difference among sulcus bleeding index of the teeth with amalgam cores and the same teeth in the opposite sides (P〉0.05).Conclusions Bonded amalgam core is preferable to preserve elderly patients' endodontically treated posterior teeth.
