摘要
目的检测原发性高血压(EH)病人之健康子女的胰岛素抵抗(IR)、血浆血管紧张素Ⅱ(AngⅡ)及降钙素基因相关肽(CGRP)的变化,探讨其在高血压发病机制中的作用以及在高血压一级预防中的意义。方法随机选择60例具有高血压家族史的健康子女,其中父亲或母亲一方患高血压的健康子女30例为单亲组,双亲均患高血压的健康子女30例为双亲组,无高血压家族史的健康子女30例为对照组。用放免法测定空腹血浆胰岛素(FINS)、AngⅡ和CGRP的含量,计算胰岛素抵抗指数(IRI)。结果双亲组和单亲组IRI、血浆AngⅡ及CGRP含量均高于对照组,差异具有显著性(F=14.43~16.63,q=6.48~8.15,P<0.01);双亲组血浆AngⅡ和CGRP含量高于单亲组,差异有统计学意义(F=15.14、16.63,q=3.12、3.98,P<0.05)。两研究组血浆AngⅡ与CGRP含量呈正相关(r=0.682,P<0.01)。结论具有高血压家族史的健康子女在血压升高之前已经存在IR等代谢改变,血浆AngⅡ及CGRP等血管活性物质的升高可能是高血压的始动因素;发现IR、血浆中AngⅡ及CGRP水平升高的有高血压家族史的健康子女,可能是高血压的高危对象,应积极采取一级预防措施防止高血压的发生。
Objective To study the changes of insulin resistance (IR) and the plasma levels of AngⅡ and CGRP in healthy children with a family history of essential hypertension (EH) and explore their effects on pathogenesis of EH and significance of its first-level prevention. Methods Sixty healthy people with a family history of EH were recruited and randomized to two groups: 30 with one parent (group A) and 30 with both parents (group B) had a history of EH, and 30 subjects without a family history of EH as controls (group C). FINS, plasma AngⅡ and CGRP were determined by radioimmunoassay, and IRI calculated. Results The levels of IR, AngⅡ and CGRP increased significantly in groups A and B compared with those in the control (F= 14.43-16.63 ,q= 6.48-8.15, P〈0.01). Plasma AngⅡ and CGRP were higher in subjects in group B than that in group A (F= 15.14,16.63;q= 3.12,3.98;P〈0.05). CGRP was positively correlated with AngⅡ in both groups A and B (r=0. 682, P〈 0.01). Conclusion The higher levels of IR and plasma AngⅡ and CGRP may exist before the development of hypertension in healthy offspring with a family history of EH, the above items are regarded as initiating agents of EH. People with IR and high levels of plasma AngⅡ and CGRP with a family history of ED are at high risk to have hypertension, a primary prevention should be offered to them.
