参麦丹参注射液联合低分子右旋糖酐治疗妊娠晚期羊水过少临床分析 被引量:12

Clinical analysis on effect of Shenmai injection and composite salvia miltiorrhizae injection combined with dextran-40 on oligohydramnios in third trimester of pregnancy

摘要 目的:探讨参麦丹参注射液联合低分子右旋糖酐治疗妊娠晚期羊水过少的临床疗效。方法:选择孕晚期未孕足月羊水过少孕妇60例,随机分为两组,每组各30例,观察组给予静滴低分子右旋糖酐加复方丹参注射液、葡萄糖注射液加参麦注射液、林格氏液;对照组给予静滴低分子右旋糖酐加复方丹参注射液、葡萄糖注射液加维生素、林格氏液。治疗前、后以多普勒超声检测AFI、S/D值,观察不同用药对剖宫产率、新生儿出生状况及新生儿体重的影响。结果:治疗后,两组AFI值均明显提高(P<0.01),而观察组AFI值明显高于对照组(P<0.01);治疗后,两组S/D值均明显降低(P<0.01),而观察组S/D值明显低于对照组(P<0.05);观察组剖宫产率为16.67%,对照组剖宫产率为26.67%;观察组新生儿Apgar评分均值明显高于对照组(P<0.05),而新生儿平均体重也明显高于对照组(P<0.01)。结论:在提高AFI值、降低S/D值、改善新生儿出生状况、增加新生儿体重方面,参麦丹参注射液联合低分子右旋糖酐治疗妊娠晚期羊水过少疗效显著,值得临床推广。 Objective : To explore the clinical curative effects of Shenmai injection and composite salvia mihiorrhizae injection combined with dextran -40 on oligohydramnios in third trimester of pregnancy. Methods: 60 oligohydramnios pregnant women in third trimester of pregnancy were selected and divided into observation group (30 cases) and control group (30 cases) stochastically. The observation group was treated with dextran -40 injection added composite salvia mihiorrhizae injection, the glucose injection added Shenmai injection and ringer's solution; the control group was treated with dextran-40 injection added composite salvia mihiorrhizae injection, the glucose injection added vitamins and ringer's solution. The amniotic fluid index (AFI) and S/D value was assessed by pulsed Doppler echocardiography before and after therapy, the rate of cesarean section, neonatal conditions and neonatal weight were compared. Results: AFI values after treatment in two groups were higher than that before treatment ( P 〈 0. 01 ), and AFI value in observation group was higher than that in control group (P 〈0. 01 ) ; S/D value after treatment in two groups were lower than that before treatment (P 〈0. 01 ), and S/D value in observation group was lower than that in control group ( P 〈 0. 01 ) . The rates of cesarean section in observation group and control group were 16.67% and 26. 67% , respectively. The Apgar value and average weight of neonates in observation group were higher than those in control group (P 〈 0.05 ) . Conclusion: Shenmai injection and composite salvia mihiorrhizae injection combined with dextran -40 can heightening the AFI value, reducing the S/D value, improving the neonatal conditions, increasing the neonatal weight, which plays a role in the treatment of oligohydramnios.
