摘要
目的观察乌司他丁对全麻单肺通气(OLV)肺叶切除术患者肺缺血-再灌注损伤的保护作用。方法全麻下OLV肺叶切除术患者20例,随机均分为观察组(A组)和对照组(B组)。A组OLV开始后,静脉泵注乌司他丁10000U/kg,1h内注完;B组给予同量生理盐水。胸内手术操作结束恢复双肺通气(TLV)。两组分别在麻醉前(T0)、手术开始0.5h(T1)、1h(T2)、2h(T3)、术后1h(T4)、2h(T5)、24h(T6)、48h(T7)8个时点采血行多形核白细胞(PMN)计数,测黄嘌呤氧化酶(XOD)和髓过氧化物酶(MPO)活性。结果两组PMN计数差异无统计学意义。与T0比较,两组PMN计数在T4~T7时明显升高(P<0.05)。A组XOD和MPO活性均明显小于B组(P<0.05或P<0.01)。结论乌司他丁减少OLV患者血XOD、MPO活性的升高,可对肺缺血-再灌注损伤起保护作用。
Objective To investigate the protective effects of ulinastatin on ischemia- reperfusion injury in patients undergoing one lung ventilation (OLV). Methods Twenty patients undergoing pulmonary resection with OLV were randomly divided into two groups with 10 cases each. The patients in group A were infused ulinastatin 10 000 U/kg during OLV and those in group B same volume normal saline. Blood samples were taken before anesthesia(T0 ), at 0.5 h(T1 ), 1 h(T2 ), 2 h (T3) during operation and for 1 h (T4), 2 h (T5), 24 h (T6), 48 h (T7) after operation for the measurements of xanthineoxidase (XOD) and myeloperoxidase (MPO) activity. The count of polymorph nuclear leukocyte (PMN) was performed as well. Results The count of PMN was significantly higher at T4-T7 than that at T0 in both groups (P〈0.05). The activities of plasma XOD and MPO were significantly higher in group B than those in group A (P〈0.05 or P〈 0.01). Conclusion Ulinastatin may protect the lung from ischemia-reperfusion injury in patients undergoing OLV through decreasing the activity of plasma XOD and MPO.
