异体脱蛋白骨关节移植的初步实验与临床应用 被引量:14

摘要 为了寻找一种具有良好生物相容性、无免疫性、能获得较好效果的生物性人工关节材料,我们在成功研制脱蛋白骨的基础上,对异体脱蛋白骨关节移植的实验与临床应用进行了初步探索,获得了比较满意的效果,大白鼠实验表明,术后4周,移植的脱蛋白骨关节内有明显血管增生,骨端有软骨样细胞出现.8、12周各有一只实验动物有成熟骨小梁及骨髓造血现象,未见炎性细胞及坏死。临床应用25例,男16例,女9例,年龄17岁~45岁,拇再造4例,半关节移植19例,骨段移植2例,随访6月到3年,2例关节脱位,3例因皮瓣坏死、感染而行移植物部分切除,植骨4月~6月愈合,关节活动增加45°~60°.手术成功的关键是解剖复位,可靠固定,良好的软组织复盖,严格控制感染及适当的关节活动。我们认为,在没有更为合适的人工关节材料时,脱蛋白异体骨关节移植不失为一种可供选择的手术方式,尤其是手部。 Study of the biomaterial about artificical articulation were rare. On the basis of succeeded in deproteinized bone. the experiments and clinical applications about deproteinized Homogeneousbane and Joint grafted were reseached preliininaril}.. The deproteinized allogenic bone and joint of the ratwas grafted in this experiment. The results of the histological examination showed obvious angiogenesisand chondroid cell were appeared during the 4 weeks. Chondrogenesis was clear and might find thematUred trabecula and bone marrow at 8 - 2 weeks.T.wenty-five patients with defects of bone or osteoarticution caused by tUmor resection or traumawere successful treated with deproleinized bone and joint allografts. There were 16 males and 9 femalesin this group of 25 cases. Their age ranged from 17 to 45 with an average of 31 .5. Of all the 25 cases, 4cases thumb reconstructed. 19 cases hendarticulation grafted, 2 cases segment of bone grafted. Thefollow up period ranged from 6 months to 3 years. The most of them has been bone union on 4 -6months. The range of the joint movement was 450 - 600 more than that of the preoperation. The keys toachieve successful were anatomic reduction, reliable fixation, satisfying soft tissue repairing, controlledinfection and appropriate joint movement. The deproteinized homogeneous bone and joint will be a newvaluable biomaterial for osteoarticular grafting. especial in hand.
