Implantation of nitinol stents and radioactive ^(125)I seeds through bronchoscopy to treat airway stenosis by advanced lung cancer

摘要 目的探讨经支气管镜置入支架及植入125I放射性粒子治疗晚期肺癌中心气管狭窄的疗效及可行性。方法对15例晚期肺癌中心气管狭窄患者,先采用支气管镜直视下置入国产镍钛记忆合金支架到狭窄部位,然后通过支气管镜把125I放射性粒子植入到气管狭窄部位的管壁、管壁外肿瘤组织。结果15例共置入17枚支架,均一次性成功置入,共植入125I放射性粒子35粒。治疗后狭窄管腔均明显扩大,气急减轻,随访3~13个月,均见支架扩张好、无移位,未见支架内及上下缘肿瘤生长导致气管狭窄。15例均未出现严重并发症。结论通过支气管镜置入支架联合125I放射性粒子植入疗效肯定、操作安全,可用于晚期肺癌中心气管狭窄的姑息治疗。 [Objective] To evaluate the efficacy and feasibility of implantation of nitinol stents and radioactive 125I seeds through bronchoscopy for treatment of center airway stenosis by advanced lung cancer. [Methods] Implantation of nitinol stents and then implantation of radioactive 125I seeds were applied to 15 cases of NSCLC with center airway stenosis. Domestic stents were used, and the release of the stents was under direct visual monitoring through bronchoscope. Radioactive 125I seeds were implanted in airway wall or outside airways. [Results] 17 stents were inserted, and 35 seeds were implanted. All of the interventions were successful. Immediate relief of dyspnea was achieved in all the patients, and the dyspnea index improved significantly after implantation. Tumor ingrowth was not found in all the patients in follow-up period. [Conclusion] Implatation of nitinol stents and radioactive 125I seeds by bronchoseopy is a safe method, and can improve the quality of life for lung cancer patients with center airway stenosis.
