CT虚拟结肠镜在结直肠癌中的临床应用 被引量:3

The Clinical Application of CT Virtual Colonoscopy to Colorectal Cancer

摘要 [目的]评价CT虚拟结肠镜(CTVC)技术在结直肠癌中的临床应用价值。[方法]96例结直肠癌患者清洁肠道后,应用GELIGHT SPEED64层螺旋CT进行容积扫描,运用CTVC技术当中的容积再现(VR)、导航技术、多平面重建(MPR)、360°大视角解剖视图、透明技术(RaySum)对图象进行后处理,并将结果与常规结肠镜(CC)、结肠气钡双重对比造影(DCBE)比较。对57例直肠癌患者,在肛缘以金属条作标记,在360°无折叠视图上测量病变下缘距肛缘距离。[结果]CTVC对结直肠癌占位诊断灵敏度93.75%;特异度92.60%;假阳性率7.41%;假阴性率6.25%;准确率93.50%;阳性预测值97.83%;阴性预测值80.65%。CTVC与纤维结肠镜相比检查成功率差别有统计学意义(P<0.05)。CTVC、CC及DCBE之间以及分别与术中所见比较其肿瘤病灶数,肿瘤沿肠管纵径及大体形态分型的结果差别无统计学意义(P>0.05)。CTVC运用360°大视角解剖视图较准确地测量了肿瘤距肛门的距离,CTVC所测距离与术中所见及直肠指检结果比较差别无统计学意义(P>0.05);纤维结肠镜所测距离与CTVC及术中所见结果比较差别有统计学意义(P<0.05)。[结论]CTVC能获得准确详细的肿瘤肠腔内外及肿瘤远近端信息,并且可反复观察,尤其适用于临床上对不能耐受结肠镜检查,肠腔高度狭窄结肠镜不能通过,尚能行肠道准备的结直肠癌患者。还能较为准确的测量直肠肿瘤下缘距肛缘的距离,指导选择手术方式。
