波前像差引导的准分子激光原位角膜磨镶术后视觉质量评价

摘要 目的比较波前引导的准分子激光原位角膜磨镶术(LASIK)与标准LASIK治疗近视眼的术后视觉质量。方法选取接受Zyoptix波前引导LASIK手术的近视患者80例(149只眼)作为试验组,并选取同期行标准LASIK手术的近视患者100例(176只眼)作为对照组,观察对比2组术后视觉质量。结果①视力变化:术后6个月,试验组最佳矫正视力较术前提高1行以上的患眼为76眼(50.1%),明显高于对照组的53眼(30.1%,P<0.01);裸眼视力高于术前矫正视力的患眼为53眼(35.6%),明显高于对照组的31眼(17.6%,P<0.01)。②屈光度变化:术后3个月对照组开始出现屈光度回退呈轻微欠矫,而试验组回退很少,基本趋于正视状态。③对比敏感度变化:术后1个月,2组对比敏感度较术前均下降,尤其以中频和高频下降明显;术后3个月,2组对比敏感度较1个月时略有增加,试验组增加明显。术后6个月,试验组对比敏感度较术前有提高,而对照组趋于术前水平。④高阶像差变化:术后2组的高阶像差均方根值(RMS)均较术前增加,但试验组较对照组增加值明显减小(P<0.05)。结论Zyoptix波前引导LASIK治疗近视的术后视觉质量优于标准LASIK手术。 Objective To evaluate the vision quality following wavefront-guided LASIK surgery vs Planoscan LASIK surgery for myopia. Methods A total of 149 eyes received wavefront-guided LASIK(Zyoptix) ,and at the same time 176 eyes were subjected to Planoscan LASIK. The vision qualities after surgery were compared. Results ①The patients subject to Zyoptix obtained better postoperative uncorrected vision acuity and better corrected vision acuity than those subject to Planoscan LASIK;② Three months after operation,the postoperative refractions of two groups began to decline, while those in Zyoptix group were declined more slowly than those in Planoscan LASIK group;③ In the early stage, the contrast sensitivity(CS) of the two groups was worse than their preoperative level. In the advanced stage,Zyoptix patients had better CS than preoperation, while Planoscan patients got closer to preoperative level.④ The higher order RMS of Planoscan patients was increased obviously as compared with that in Zyoptix patients. Conclusion Patients undergoing Zyoptix have better quality of vision than those undergoing.Planoscan LASIK.
