摘要
目的观察采用微量注射泵泵入硝酸甘油(NTG)注射液和磷酸二酯酶抑制剂米力农治疗慢性肺源性心脏病(CPHD)的临床疗效。方法把CPHD患者随机分为两组,治疗组34例,在CPHD常规治疗基础上每日加用微泵泵入NTG和米力农注射液各5 mg/d,均加0.9%氯化钠注射溶液至50ml,分别按5-10μg/min,0.25-1.0μg·min^-1·kg^-1泵入,疗程一周。对照组27例,为CPHD常规综合治疗。观察治疗前后各组的临床疗效,对动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、血氧饱和度(SaO2)、平均肺动脉压(MPAP)、左室射血分数(LVEF)监测。结果治疗组与对照组在住院7 d内治疗总有效率分别为96%与78%(P〈0.01),治疗组显效率更高(P〈0.01)、降低MPAP、PaCO2水平更显著(P〈0.01)。结论微量泵入硝酸甘油和米力农对慢性肺源性心脏有明显疗效,且无明显不良反应。
Objective To observe the clinical effect of Nitroglycerin (NTG) plus milrinone injection by microscale injector pump im treating patients with chronic pulmonary heart disease. Methods 62 patients with chronic pulmonary heart disease were randomly divided into treatment group (n = 34, NTG injection 5mg/d for 5 - 10μg/min plus Milrinone injection 5mg/d for 0. 25 - 1.0 μg/( min · kg) by microscale injector pump) and control group ( n = 27,conventional therapy ). The whole therapeutic course consisted of 7 days, and PaO2, PaCO2 , SaO2, MPAP and LVEF were monitored. Results The effective rate (96%) in treatment group was significantly more than that 78% in control group (P 〈 0.01 ). MPAP and PaCO2 level in treatment group were markedly lower than those in control group (P 〈 0.01 ). Conclusion The effect of NTG plus Milrinone injection by microscale injector pump is remarkable and safe for treating patients with chronic pulmonary heart disease.
