摘要
目的探讨p53和c-erbB-2的表达阳性率与胃癌浸润深度的关系。方法收集139例胃癌手术切除标本,包括黏膜内癌23例、浸润至黏膜下层31例、浸润至肌层47例、浸润至浆膜或浆膜外38例,切片采用免疫组化S-P法染色。结果p53和c-erbB-2的阳性表达分别从肿瘤局限于黏膜内和侵及黏膜下层时开始出现。不同浸润深度的胃癌中p53的阳性率比较,仅黏膜内癌组和浸润至浆膜或浆膜外组(P<0.01)及浸润至黏膜下层组和浸润至浆膜或浆膜外组存在显著差异性(P<0.05),其余均无统计学意义。而各组c-erbB-2的表达阳性率比较均存在显著差异性(P<0.01)。c-erbB-2表达阳性率与p53存在相关性(P<0.05)。p53和c-erbB-2的不同表达方式中,浸润深度p53+c-erbB-2+组或p53-c-erbB-2+组>p53+c-erbB-2-组>p53-c-erbB-2-组(P<0.05),但p53+c-erbB-2+组与p53-c-erbB-2+组比较无统计学意义(P>0.05)。结论p53和c-erbB-2在胃癌中的表达不是同步的,p53突变有利于c-erbB-2的表达。在胃癌的浸润中,癌基因c-erbB-2突变或者抑癌基因p53突变是独立发挥作用的,并且二者没有明显的协同作用;表达c-erbB-2的胃癌病例比表达p53的病例更具有侵袭性。
Objective To explore the positive rates of p53 and c-erbB-2 expression in gastric cancer and their relationship with tumor invasion. Methods SP immunohistochemistry was used to detect the positive rates of 1353 and c-erbB-2 expression in 139 gastric cancer excisional speciments. Results The expression of p53 and c-erbB-2 in gastric cancer started respectively at the time when tumor was bruited in the mucosal tissue and invaded the submucosa. Comparing the positive rates of p53 in gastric cancer with different invading depths, only the distinction between the group of tumor invading or exceeding serous membrane and the group of mucosal carcinoma ( P 〈 0. 01 ) or the group of submucous carcinoma ( P 〈 0. 05) was obvious. The others were not statistically significant. But the positive rate of c-erbB-2 in each group was dissimilar evidently (P 〈 0. 01 ). The relation of the positive rates between c-erbB-2 and p53 was significant ( P 〈 0. 05 ). When p53 and c-erbB-2 were expressed in gastric cancer in different manners, the depth of invasion was distinct:the group of 1353 + c-erbB-2 + or p53 -c-erbB-2 + 〉 the group of p53 + c-erbB-2 - 〉 the group of p53 - c-erbB-2 - ( P 〈 0. 05 ). But the distinction between the groups of p53 + c- erbB-2 + and p53 - c-erbB-2 + was not statistically significant ( P 〉 0.05 ). Conclusion The expression of p53 and c-erbB-2 expression in gastric cancer were coincident. The mutation of oncogene c-erbB-2 or anti-oncogene p53 may have independent role in gastric cancer invasion, and there is not synergism between them. Then gastric cancer cases expressed by c-erbB-2 were more invasive than those expressed by p53.
