摘要
目的观察慢性心力衰竭(心衰)患者左室射血分数的降低与凝血功能、血浆N-末端前体脑钠肽(Nt-proBNP)浓度之间的关系。方法测定79例慢性心衰患者左室射血分数、Nt-proBNP浓度及凝血功能,观察慢性心衰患者左室射血分数的降低与凝血功能及Nt-proBNP浓度的变化关系。结果慢性心衰患者左室射血分数与Nt-proBNP浓度呈负相关,随着左室射血分数的降低,Nt-proBNP浓度逐渐增高;慢性心衰患者凝血功能与对照组相比,凝血酶原时间(PT)、凝血酶时间测定(TT)、活化部分凝血酶原时间(APTT)缩短,而纤维蛋白原(FIB)延长;且随着慢性心衰患者左室射血分数的逐渐降低凝血功能中PT和APTT有延长趋势,FIB有缩短趋势。结论慢性心衰患者血浆Nt-proBNP的浓度与左室射血分数呈负相关,血中Nt-proBNP浓度能直接反映心功能损伤或衰竭的程度;慢性心衰患者的凝血功能呈相对激活状态,易促使血小板的粘附、聚集及血栓前状态的形成。
Objective To observe correlation among left ventricular ejection fractionin (LVEF), coagulation function and Nt - proBNP concentration of patients with chronic cardiac failure ( CCF ). Methods Seventy - nine cases with CCF were detected for LVEF, Nt - proBNP concentration and coagulation function, and relationship among LVEF decrease, coagulation function and Nt -proBNP concentration changes was observed. Results LVEF of CCF patients negatively correlated with Nt - proBNP concentration (r = - 0. 569 4, P 〈 0. 05 ), and Nt - proBNP concentration gradually increased along with LVEF decreased ( P 〈 0.05 ). Compared with the control group, PT, TT and APTT of the CCF patients were shorter ( P 〈 0.1 ), while the FIB extended ( P 〈 0.1 ), and there was tendency in PT and APTT prolongation ( P 〈 0.1 ) and FIB decurtation (P 〈 0.1 ) along with LVEF reduction. Conclusions Plasma Nt - proBNP concentrations of CCF patients negatively correlates with LVEF. Blood Nt -proBNP concentration can directly reflect the hazard and failure degree of cardiac function. The coagulation function of CCF patients shows relative state of activation that can easily promote the thrombocytic adhesion and aggregation, and formation of pre - thrombosis state.
