血清降钙素原和C-反应蛋白在细菌感染性疾病诊断中的意义 被引量:54

摘要 目的评价血清降钙素原(PCT)和C-反应蛋白(CRP)对细菌感染性疾病诊断的临床价值。方法112例重症监护病房(ICU)细菌感染的患者作为感染组,并以110例ICU非细菌感染性患者作为对照组,测定并比较分析两组患者血清PCT和CRP结果。结果①感染组血清PCT水平显著高于对照组(P<0.05);感染组的PCT检测阳性率为94.6%,高于对照组(16.4%)(P<0.05);②感染组血清CRP水平显著高于对照组(P<0.05);感染组的CRP检测阳性率为31.3%,高于对照组(28.2%),但两组差异无统计学意义(P>0.05);③PCT检测对细菌感染性诊断的灵敏度、特异度、阳性预测值、阴性预测值和诊断符合率均高于CRP检测结果,相比较有显著性差异(P<0.05)。结论PCT对于严重细菌感染性疾病的诊断较CRP敏感,监测血清PCT水平可以作为严重细菌感染的特异性监测指标,对临床诊治具有指导意义。 Objective To evaluate clinical value of serum procalcitonin and C-reactive protein in diagnosis of bacterial infections. Methods 112 cases of intensive care unit (ICU) patients with bacterial infections as the infection group,and 110 cases of non-bacterial infection of ICU patients as control group, the two groups of serum PCT and CRP results were determined and a comparative analysis. Results (1) The infection group serum PCT level was significantly higher than that of the control group(P〈0.05);the positive rate of PCT was 94.6% in the infection group, which was higher than that of the control group(16.4%) (P〈0.05);(2)Serum CRP level in the infection group was significantly than that of the control group(P〈0.05). The positive rate of CRP was 31.3% in the infecion group, control group (28.2%), but no significant difference between the two groups (P〉0.05);(3) The sensitivity,specificity,positive predictive value, negative predictive value and diagnostic rate in the PCT diagnostic for detection of bacterial infection was higher than that of the CRP, the difference was significant (P〈0.05). Conclusion PCT for serious bacterial infections sensitive diagnosis is higher than that of the CRP, monitoring serum PCT levels can be used as a specific indicator of serious becterial infection, which has guiding significance for clinical diagnosis and treatment.
