摘要
目的:为探讨Ptfv1对诊断慢性肺心病右房肥大和右心功能不全的临床价值,方法:对126例肺心病患者及104例正常人的常规心电图P波进行分析,并对Ptfv1异常者进行动态心电监测。对其中70例患者进行了超声心动图检查。结果:肺心病加重期Ptfv1阳性率38.88%,缓解期转阴率36.72%,改善率40.81%,总好转率77.53%。两期比较差异有高度显著性(P<0.001)。肺型P波阳性率46.3%,与Ptfv1比较差异无显著意义(P>0.05)。正常人对照组未发现Ptfv1阳性及肺型P波。超声心动图显示右房内径增大率为71.43%,其中Ptfv1阳性78%,肺型P波88%。结论:提示肺型P波和Ptfv1异常的临床符合率较高,两者在诊断价值上可能是一致的,提示Ptfv1<-0.02mm·s可作为判定慢性肺心病右房肥大的诊断标准之一。Ptfv1值随右心功能状态变化而变化,故Ptfv1值动态变化可作为右心功能不全的诊断指标之一。
Objective:To discuss the clinical value of Ptfv1 in diagnosing right atrial hypertrophy and right heart failure in chronic pulmonary heart disease.Methods:Routine electrocardiograms (ECG) of 126 pulmonary heart disease patients and 104 normal adults were analysed and of which abnormal Ptfv1 was dynamically monitored:Doppler examinations were performed on 70 cases of the studied patients.Results:In the progressive stage of pulmonary heart disease. The positive rate of Ptfv1 was 38.88%;and in the recovering stage the negative reversing rate was 36.72%;The improving rate was 40.81%.The total improving rate was 77.48%.There was significant difference between the two stages (P<0.001) .The positive rate of pulmonary p-wave was 46.3%.and was no significant difference from Ptfv1 positive rate (P>0.05) .There were no positive Ptfv1 and pulmonary p-wave in the normal control group.The rate of enlarged right atrium detected by doppler was 71.43%.of which positive Ptfv1 rate was 78% and the rate of pulmonary pware was 88%.Conclusions:Ptfv1 abnormalities correspond with pulmonary pwave in high propotion and it is considered that they are of the same diagnostic value.It is also indicated that Ptfv1 <-0.02mm.s could be one of the diagnostic standards for right atrival hypertrophy in pulmonary heart disease.Ptfv1 changes in pulmonary heary disease ptfVl changes with right heart function.so dynamic changes of Ptfv1 could be one of the diagnostic datae for right heart function.
