手术治疗老年骨质疏松胫骨平台骨折37例分析

摘要 目的分析不同的手术内固定方法治疗老年骨质疏松性胫骨平台骨折的临床疗效。方法2006年1月至2008年3月对37例老年骨质疏松性胫骨平台骨折患者行手术治疗,依据Schatzker分型:Ⅱ型9例,Ⅳ型6例,Ⅴ型12例,Ⅵ型10例。合并交叉韧带损伤4例,半月板损伤12例。结果随访9~18个月所有患者均获得骨性愈合,依据HSS膝关节功能评定结果:优21例、良8例、可5例、差3例,优良率达78.4%。结论不同的手术内固定方法治疗老年骨质疏松性胫骨平台骨折临床疗效满意,尤其对老年骨质疏松性胫骨平台骨折,能够牢靠维持复位,有利于功能锻炼,促进骨量的增加加快愈合。 Objective To analyze the clinical effect of the tibial plateau fractures with olderly osteoporosis with all kinds of operation. Methods Thirty-seven cases of fracture with osteoporosis of the tibial plateau were treated with operation from July 2006 to May 2008. according to schatzker classification 9 cases were type Ⅱ, 6 cases type Ⅳ, 12 cases type Ⅴ, 10 cases type Ⅵ, and 4 cases with cruciate ligament injury, 12 cases with meniscus injury. Results All the patients who followed up for 9 to 18 months obtained satisfactory curative effect, the clinical outcome were evaluated according to HSS grading, 21 cases were graded as excellent and 8 cases as good, 5 cases were canbe, 3 cases were poor. The excellent rate was 78.4 %. Conclusion Operation has been proven as a safe and effective the tibial plateau fractures with olderly osteoporosis, and facilitate early postoperative rehabilitation, promote bone mass increase and speed up fracture healing.
