胃大部切除术后不同营养支持方法的比较及护理体会 被引量:7

摘要 目的比较肠外营养支持与早期肠内营养支持对胃大部切除术后患者营养状况的影响,并探索其科学合理的护理方法。方法将2004年10月至2008年10月行胃大部切除手术的患者86例随机分成对照组41例和实验组45例,对照组通过静脉输液进行肠外营养支持,实验组在术后16~20小时开始进行肠内营养支持,两组患者每日给予的热量、氮量相等。比较两组患者的营养指标、并发症的发生率及住院天数。结果实验组患者术后8天的营养指标高于对照组(P〈0.01),并发症总发生率与对照组比较差异有统计学意义(P〈0.05),住院天数比较差异无统计学意义(P〉0.05)。结论早期肠内营养支持结合科学合理的护理方法,能提高胃大部切除患者的营养指标,降低并发症的发生率,提高患者的生存质量。 Objective To compare the effect of parenteral nutrition with early enteral nutrition on patients nutritional status after subtotal gastrectomy, and search for the scientific and equitable nursing ways. Methods 86 patients suffered subtotal gastrectomy were randomly divided into the control group (n=41) and the experimental group (n=45). The control group received parenteral nutritional supply by venous transfusion. The experimental group received early stage enteral nutritional supply during the 16-20 hours after operation. Both groups were accepted the equal quantity of heat and nitrogen. The nutritional conditions, complications and hospital stay between them were observed and analyzed. Results At 24 hours after operation, all the nutritional parameters in the experimental group were better than that in the control group (P〈 0.01). There was statistic difference in total complication incidences between the two groups(P 〈 0.05), but no in hospital stay (P 〉 0.05).Conclusion The early enteral nutrition after operation and elaborate nursing care can improve nutritional condition, reduce the incidence rate of complication and raise the life qualities of patients.
