摘要
按统一标准选取风湿性心脏病患者１２０例，随机分为正常对照组、气阳虚证组及气阴两虚证组，后两组分别给予温阳益气佐以活血、利水及益气养阴佐以活瘀方剂治疗，分别于治疗前及治疗６周后采集血样，测定免疫指标并观察患者症状、体征变化情况。结果表明：风心病气阳虚和气阴虚患者免疫功能呈紊乱状态，提示风心病存在自身免疫病的倾向。所有指标水平在两组间的异常变化相一致，证实了气虚乃是两组共同的发病机制，认为扶正固本，用温阳益气或益气养阴，辅以活瘀、利水，标本兼顾，增强细胞免疫抑制亢进的体液免疫是治疗风心病的基本法则。益气、温阳、养阴药物可能在增强细胞免疫功能中起主导作用，而活瘀利水药在抑制亢进的体液免疫方面作用突出。
Abstract According to the integrate standard,120 cases of rheumatic heart disease patients were selected and divided into three groups:controll group,group of deficiency syndrome of qi and yang and group of deficiency syndrome of qi and yin .The latter two groups are treated respectively with the prescription of warming yang to supplement qi assisted with medicines of promoting blood flow for diuresis and with that of supplementing qi and nouishing yin assisted with medicines of removing blood stasis.The blood samples were collected from the two groups respectively before the treatment and six weeks after the treatment. We tested the immunological index and observed the patients syndrome and physical changes.The result suggested that both the immunological functions of the patients of the rheumatic heart disease with deficiency of qi and yang and those with deficiency of qi and yang were in disorder,and the rheumatic heart disease itself is apt to show the immunological trouble.The coherence of unusual changes of the index level in the two groups proves that the difficiency of qi is the common pathogenesis.It is considered that the fundamental principle for the treatment of rheumatil heart disease is supporting vital qi to reinforce the origin,warming yang to supplement qi ,supplementing qi and nourishing yin assisted with removing blood stasis for diuresis,treating both the symptoms and causes of a disease at the same time,and to restrain the immunological function,and to restrain the immunological function of the stimulating body fluid.To sum up,the medicine of supplementing qi ,warming yang and nourshing yin may play a key role in strengthening the cells immunological functions,while the medicine of removing blood stasis for diuresis may play a pecular part in restraining the immunological function of the stimulating blood fluid.
