摘要
目的采用6min步行试验,观察充血性心力衰竭(CHF)患者运动前后血清肿瘤坏死因子α(TNF-α)和白细胞介素10(IL-10)水平的变化。方法34例CHF患者和22例健康对照者进行了6min步行试验(6-MWT),于运动前后采取静脉血,使用双抗体夹心ABC-ELISA法测量血清IL-10和TNF-α的浓度。结果CHF组基础状态的血清TNF-α水平显著高于对照组(P<0.01),而IL-10水平和6min步行距离(6-MWTD)均低于对照组,但IL-10的变化差异无统计学意义(P>0.05)。与运动前相比,对照组运动后即刻TNF-α水平无显著性改变(P>0.05),但IL-10水平显著性升高(P<0.05);CHF组TNF-α水平运动后即刻呈明显升高(P<0.01),而IL-10水平运动后即刻并无显著意义的升高(P>0.05)。结论CHF患者较正常人运动耐量下降,一次6min步行运动使CHF患者炎症性细胞因子水平进一步增加,抗炎症性细胞因子水平不增加,从而加重了细胞因子之间的网络调控紊乱。
Objective To Study the effectof six minute walk test(6-MWT)on the levels of serum interleukin-10(IL-10) and tumor necrosis factor-α(TNF-α) in patients with congestive heart failure(CHF). Methods 34 CHF patients and 22 hehhy subjects underwent 6-MWT.The blood samples were taken before and immediately after 6-MWT from all subjects. Senan IL-10 and TNF-α levels were assessed with a sandwich ELISA assay. Results The levels of serum TNF-α were significantly higher( P 〈 0.01 ), and IL- 10 levels were lower, and 6-MWT distances were shorter( P 〈 0.01) in CHF patients than those in healthy subjects at rest, but IL-10 levels between CHF patients and healthy subjects were no significantly change( P 〉 0.05). Compared with those before 6-MWT, Serum TNF-a levels immediately after 6-MMT in healthy subjects were no significantly change( P 〉 0.05) ,but IL-10 levels were significantly higher ( P 〈 0.05 ) ; Serum TNF-a levels immediately after 6-MWT in CHF patients were obviously higher( P 〈 0.01 ), but IL- 10 levels were no significantly change( P 〉 0.05). Conclusion Exercise intolerance in CHF patients is decreased, a 6-MWT make proinilamatory cytokine levels further raised and anti-inflammatory cytokine levels relatively lowered in CHF patients, therefore, a 6-MWT may aggravate the imbalance cytokine network control in CHF patient.
