有晶状体眼后房型人工晶状体植入矫正高度近视对眼前节形态的影响 被引量:14

摘要 目的探讨有晶状体眼后房型人工晶状体植入术后眼前节结构形态及眼压的变化情况。方法选取中国医科大学附属第四医院眼科于2007年9月至2008年4月行双凹型可折叠单片型人工晶状体(implantable contact lens,ICL)植入术的30例(52眼)患者,均于术前1周及术后3个月利用超声生物显微镜对角膜中央厚度,中央前房深度,上方、下方、鼻侧及颞侧4个方向房角角度,ICL中央厚度,ICL距晶状体中央前表面的距离,晶状体中央厚度以及眼压等多个指标进行观察测量和统计学分析。结果术前及术后角膜中央厚度分别为(0.50±0.06)mm、(0.51±0.05)mm,二者相比差异无统计学意义(P>0.05);中央前房深度分别为(3.27±0.24)mm、(2.38±0.38)mm,二者相比差异有统计学意义(P<0.05);4个方向房角度数分别为:12点钟位(38.69°±12.17°、25.24°±7.10°)、3点钟位(39.49°±10.60°、22.36°±5.11°)、6点钟位(39.11°±9.02°、23.32°±5.26°)、9点钟位(41.32°±11.02°、23.38°±6.22°),术前及术后4个方向房角角度相比,差异均有统计学意义(P<0.05);晶状体中央厚度分别为(3.47±0.24)mm、(3.59±0.21)mm,眼压分别为(16.74±2.37)mmHg(1kPa=7.5mmHg)、(16.78±3.56)mmHg,二者相比差异均无统计学意义(P>0.05)。ICL中央平均厚度为(0.21±0.05)mm,距晶状体中央前表面的平均距离为(0.54±0.28)mm。结论有晶状体眼后房型人工晶状体具有矫正效果好、预测性好、可逆性等优点,初步临床观察显示近期效果较满意。 Objective To investigate the morphological changes of anterior segment and intraocular pressure(IOP) in phakic eyes after posterior chamber IOL implantation. Methods Thirty patients(52 eyes) treated with implantable contact lens(ICL) in the Department of Ophthalmology, the Fourth Affiliated Hospital of China Medical University from September 2007 to April 2008 were studied. Ultrasound biomicroscopy (UBM) examination was used to measure central anterior chamber depth, chamber angle degree at the top, bottom, nasal and temporal sides, ICL central thickness, ICL central surface distance from the lens, thickness of the lens and IOP at preoperative 1 week and postoperative 3 months, respectively. All the results were analyzed by SPSS 11.5. Results Preoperative and postoperative average central corneal thickness were (0. 50 ± 0. 05) mm and (0. 51 ± 0. 05 ) mm, respectively ( P 〉 0.05 ) ; Preoperative and postoperative average central anterior chamber depth were ( 3.27 ± 0.24 ) nun and ( 2.38 ± 0.38 ) mm (P 〈 0. 05 ) ; Preoperative and postoperative chamber angle degree were 38. 59° ± 12. 17° ,25.24° ±7. 10°at 12 clock site,39.49° ± 10. 60° and 22.36°± 5. 11° at 3 clock site,39.11° ± 9.02° and 23.32°± 5.26° at 6 clock site,41.32° ± 11.02° and 23.38° ± 6.22° at 9 clock site,respectively. There was significant difference with chamber angle degree at 4 sides between pre- and post- operation ( all P 〈 0. 05 ) ; Preoperative and postoperative lens central thickness were ( 3.47 ± 0.24) mm and ( 3.59 ± 0.21 ) nun, respectively; Preoperative and postoperative IOP were ( 16.74 ± 2.37 ) mmHg ( 1 kPa = 7.5 mmHg) and ( 16.78 ± 3.56) mmHg, respectively (P 〉 0.05 ) ;Average ICL thickness was (0. 21 ± 0.05 )mm; ICL surface distance from the lens was (0.54 ± 0.28 ) ram. Conclusion Posterior chamber IOL implantation in phakic eye has a good correction, high prediction, fine reciprocity and satisfactory effect with clinical observation.
