摘要
目的:利用血管回声跟踪技术(echotracking,ET)评价2型糖尿病(T2DM)及高脂血症患者的颈动脉弹性。方法:血脂水平增高(T2DM组1)和血脂水平正常(T2DM组1)T2DM患者分别为39和21例,正常对照60例,单纯高脂血症组30例。应用ET技术检测颈动脉硬化参数,并进行统计学分析。结果:T2DM组1与T2DM组2、高脂血症组压力-应变弹性系数(Ep)、硬度指数(β)、脉搏波传导速度(PWVβ)均较正常对照组高(P<0.01),顺应性(AC)均较对照组降低(P<0.01);T2DM两组β、Ep、PWVβ均较高脂血症组高(P<0.05),AC、膨大指数(AI)差异不明显(P>0.05);T2DM组2β、Ep、PWVβ较T2DM组1升高(P<0.05),AC、AI差异不明显(P>0.05)。结论:合并高脂血症的T2DM患者动脉硬化更明显,ET技术能在内膜增厚及斑块形成之前对T2DM患者颈动脉硬化做出准确诊断。
Objective To assess the elasticity of the common carotid artery in patients with type 2 diabetes mellitus (T2DM) and hyperlipidemia by echo-tracking (ET) technique. Methods 39 diabetic patients with normal lipid levels, 21 patients with diabetes and hyperlipidemia, 30 hyperlipidemic patients, and 60 healthy controls were enrolled in this study. ET technique was used to assess the carotid elasticity and the findings were analyzed statistically. Results As compared with the control subjects, β, Ep, and PWVJ3 were significantly elevated in all the patients (P 〈 0.01) and AC was declined (P 〈 0.01). β, Ep, and PWVβ were higher in all the diabetic patients than in the patients with hyperlipidemia alone (P 〈 0.05), while AC and AI did not differ significantly (P 〉 0.05). 13, Ep, and PWVβ were elevated more in the patients with diabetes and hyperlipidemia than in the diabetic patients with normal lipid levels (P 〈 0.05), and AI and AC did not differed obviously (P 〉 0.05). Conclusions Artery atherosclerosis is more obvious in patients with T2DM and hyperlipidemia. Echo-tracking technique can accurately predict arteriosclerosis before the intima-media thickening and plaque formation.
