冠心病发病危险因素的临床分析

Clinical analysis of major cardiovascular risk factors for coronary heart disease

摘要 目的探讨冠心病发病主要危险因素及分布特点。方法分析246例疑诊CHD患者的临床资料,对(CHD)与各危险因素关系作单因素、多因素Logistic回归分析。结果经冠状动脉造影诊断CHD172例,非CHD74例。单因素Logistic回归分析显示,年龄、高血压、糖尿病、吸烟、空腹血糖、高低密度脂蛋白胆固醇(LDL-C)血症及血脂异常的比值比(OR)>1.0(P<0.05),CHD与上述因素正相关;载脂蛋白A1(ApoA1)水平OR值<1.0(P<0.05),CHD与ApoA1水平负相关。多因素Logistic回归分析显示,依OR值高低排列,糖尿病史、高LDL-C血症、年龄、ApoA1水平入选回归方程,其中糖尿病、高LDL-C血症与年龄OR值>1.0(P<0.05),CHD与上述因素呈正相关;ApoA1水平OR值<1.0(P<0.05)。另外,患者对高脂血症知晓率低,非CHD组实际血脂异常发生率也高,与CHD组血脂异常分布相似[低高密度脂蛋白胆固醇(HDL-C)、高甘油三酯(TG)、高LDL-C]。结论糖尿病史、高LDL-C血症、年龄可看作CHD危险因素,ApoA1可视为CHD保护因素。非CHD者血脂异常发生率也高,防治工作中不可忽视。 Objective To investgate the current status of major cardiovascular risk factors for coronary heart disease(CHD). Methods The clinical data from 246 patients with suspected CHD were analyzed by logistic regression model. Results Of 246 patients,CHD was diagnosed in 172 cases and non-CHD in 74 cases according to the results of coronary artery angiography. Single factorial logistic regression analysis showed that there was a positive correlation between CHD and advanced age, hypertension, diabetes mellitus, smoking, fasting blood glucose, high low density lipoprotein cholesterol (LDL-C) level and lipidemia and low apolipoprotein-A1 (ApoA1) level (OR〉1. 0, P〈 0. 05). By multiple factor logistic regression analysis, among the final 4 factors, diabetes mellitus, high LDL-C level and advanced age were the risk factors for CHD (OR〉1, P〈0. 05) and ApoA1 was the protective factor of CHD (OR〈1.0, P〈0.05). Of all patient, the knowledge rate of lipidemia was low, the highly prevalence of current lipidemia was mainly formed by prevalence of low high density lipoprotein cholesterol (HDL-C) level, high triglycerin (TG) level, high LDL-C level in groups CHD and non-CHD. Conclusion The history of diabetes mellitus, high LDL-C level, advanced age are the the risk factors for CHD and serum level of ApoA1 may be taken as a protective factor of CHD. Since the prevalence of lipidemia is high in the non-CHD patients in the area, lipidemia should not be neglected in the prevention of CHD.
