摘要 目的:研究PTEN、Fas/FasL和DCR3在人胃癌组织中的表达,初步探讨三者与胃癌的发生、癌细胞增殖和侵袭转移的关系及其临床意义。方法:采用链霉素抗生物素蛋白-过氧化酶连接(SP)免疫组织化学方法检测75例胃癌标本黏膜组织中PTEN、Fas/FasL和DCR3蛋白的表达,15例胃溃疡或十二指肠溃疡患者胃黏膜组织作为正常对照组。结果:胃癌组织中DCR3的表达率显著高于正常胃黏膜组织,PTEN及Fas/FasL的表达率则低于正常胃黏膜组织,差异均有统计学意义(P<0.01);DCR3与Fas/FasL、PTEN的表达呈负相关(P<0.001),PTEN与Fas/FasL的表达呈正相关(P<0.001,r=0.401),三者的阳性表达率与肿瘤淋巴转移、临床分期、肿瘤病理分化程度及浸润深度密切相关,与性别、年龄及肿瘤大小无关(P>0.05)。结论:DCR3、PTEN及Fas/FasL在胃癌的发生发展及浸润转移中存在着相互制约的关系。检测三者在胃癌中的表达,有助于从不同角度判断胃癌的恶性生物学行为;DCR3、PTEN及Fas/FasL有可能成为胃癌临床诊断、判断疗效及监测预后的可靠指标。 Objective:To study the expression of PTEN,Fas/FasL and DCR3 in human gastric cancer and evaluate their effects on carcinogenesis, cell proliferation,invasion and metastasis of gastric carcinomas, as well as clinical significance. Methods: The expression of PTEN, Fas/FasL and DCR3 were detected by immunohistochemical method ( streptavidin -peroxidase) in the tissues of 75 primary gastric carcinomas and 15 gastric ulcers. Results: The positive expression rate of DCR3 in gastric carcinoma tissues was significantly higher than that in normal tissues,and the expression of PTEN and Fas/FasL was obviously lower than that in normal tissues ( P 〈 0.01 ). The expression of DCR3 was negatively correlated with the expression of Fas/FasL and PTEN respectively ( P 〈 0.001 ). There was a positive correlation between the expression of PTEN and Fas/FasL. The expression of PTEN, Fas/FasL and DCPt3 was closely correlated with lymph node metastasis, clinical stage, histological differentiation grade and tumor depth of gastric cancer,but not correlated with sex,age and tumor size (P 〉0.05). Conclusion: PTEN,DCR3 and Fas/FasL have mutual restrictive relationships in the process of invasion and metastasis in gastric cancer. Detecting the expression of PTEN, Fas/Fas and DCR3 in gastric cancer is helpful to judge the biological behavior of gastric cancer,these three indices may be considered as reliable markers in clinical diagnosis and the prognosis of gastric carcinoma.
