基于血糖负荷概念的食物交换份法用于2型糖尿病患者饮食教育的效果分析 被引量:51

摘要 目的探索基于血糖负荷(GL)概念的食物交换份法用于2型糖尿病患者饮食教育的有效性。方法在上海市某三级甲等医院糖尿病门诊抽取72例2型糖尿病患者,采用随机数字法分为试验组与对照组,每组各36例。试验组采用基于GL概念的食物交换份法,对照组采用传统的食物交换份法,教育内容上不涉及GL相关知识。两组均通过群体教育、病友会、门诊个别指导、电话咨询、发放书面教育材料等方式实施为期3个月的饮食教育干预,干预时间、干预形式和频次相同。以空腹血糖、餐后2h血糖、糖化血红蛋白、三酰甘油、总胆固醇和体质指数、腰围、血压为主要评价指标,于干预后3个月进行效果评价。结果两组空腹血糖、餐后2h血糖、糖化血红蛋白、三酰甘油、总胆固醇水平和体质指数的差异具有统计学意义(P<0.05或P<0.01)。结论在2型糖尿病患者的饮食教育中应用基于GL概念的食物交换份法,其效果优于传统的食物交换份法,值得推广。 Objective To explore the effects of the food exchange serving based on glycemic load in the diet education for patients with type 2 diabetes. Methods 72 patients with type 2 diabetes were randomly assigned into two groups. All the patients received 3-month diet education by the means of health lectures,DM patients' salon activities,outpatient-consultations, telephone-consultations and related materials. The 36 patients in the control group received diet education of traditional food exchange serving,while those in the experimental group received diet education of the food exchange serving based on glycemic load. Fasting blood glucose (FBG),2h postprandial blood glucose (2hPBG),glycosylated hemoglobin (HbAlc) ,total cholesterol(TC),triglycerides(TG) ,body mass index(BMI),waist circumference (WC) and blood pressure(BP) were measured after the intervention. Results There were significant differences in FBG,2hPBG,HbAlc,TC,TG and BMI(P〈0.05 or P〈0.01)between the two groups. Conclusion The food exchange serving based on glycemic load is more effective than the traditional food exchange serving in diet education for pa- tients with type 2 diabetes.
