摘要
作者观察了42例严重烧伤病人早期血栓素(TXA_2)、前列环素(PGI_2)、循环血小板微聚物比率(CPAR)、血小板计数、全血粘度、心肌酶谱、皮质醇和肾上腺素的动态变化,分为两组。结果发现,对照组TXA_2、TXA_2/PGI_2比值、心肌酶谱、全血粘度、皮质醇和肾上腺素均明显增高。输注氢澳酸山莨菪硷(654-2)后,654-2组上述指标均明显低于对照组,而CPAR和血小板计数则明显高于对照组。TXA_2与CPAR、血小板计数,全血粘度、心肌酶谱均呈明显相关(P<0.05~0.01)。表明TXA_2在烧伤休克血流变学变化及组织缺血损害中起一定作用,抑制剂654-2可减轻上述变化。
The serial changes in thromboxane (TXA2) prostacyclin(PGI2),circulatory platelet aggregate ratio (CPAR),platelet count,blood viscosity,myocardial enzyme spectrum,cortisol and epinephrine were determined in 42 severely burnt patients randomly divided into two groups.The findings demonstrated that in the control group,both TXA2 and TXA2/PGI2 ratio increased significantly during the early postburn stage.Myocardial enzyme spectrum,blood viscosity,cortisol and epinephrine also increased markedly.However,levels of the above parameters in the anisodamine-treated group were significantly lower than in the control following the infusion of anisodamine.On the contrary,CPAR and platelet count in the treated group increased and were significantly higher than those in the control.Moreover,TXA2 was closely correlated with CPAR,platelet count,blood viscosity and myocardial enzyme spectrum (P<0.05-0.01).The findings of the present study suggested that TXA2 was one of the important factors in initiating hemorrheological changes and ischemic damage of tissues in burn shock.Its inhibitor,anisodamine,could alleviate the hemorrheological disturbance and ischemic damage
