摘要
目的研究阿尔茨海默病伴快速眼动睡眠期行为异常患者的睡眠特点,以及临床和电生理学特征。方法3例临床诊断明确的阿尔茨海默病伴快速眼动睡眠期行为异常患者接受多导睡眠图监测,记录其脑电图、眼动图、心电图、肌电图、打鼾、鼻气流量。胸式和腹式呼吸动度及血氧饱和度等睡眠相关电生理活动,计算下颏和下肢肌电活动增多时间占快速眼动期的。比例;并记录快速眼动期异常肢体活动。结果3例患者快速眼动期睡眠时间占总睡眠时间的比例增加(29.37%~33.17%),下颏和下肢肌电活动增多,其时间分别占快速眼动期的7.19%-14.76%和0.11%-6.36%。2例患者异常肢体活动增多。结论阿尔茨海默病患者可伴快速眼动睡眠期行为异常,可能与快速眼动调节通路损害有关。
Objective To study the sleep characteristics, and clinical and electrophysioIogical features of probable Alzheimer disease (AD) associated with rapid eye movement sleep behavior disorder (RBD). Methods Three cases of probable AD associated with RBD were monitored by polysomnography (PSG). Electroencephalography (EEG), ophthalmography, electrocardiography (ECG), electromyography (EMG), snoring, nasal air flow, thoracic and abdominal breathing movement, and oxygen saturation (SaO2) were recorded. The percentages of enhanced EMG activities of chin and lower limb during rapid eye movement (REM) sleep stage were calculated. At the same time, the abnormal extremity movements during REM sleep stage were recorded. Results In 3 cases, the percentage of REM sleep stage to total sleep duration was increased (29.37%-33.17%). The percentages of enhanced EMG activites of chin and lower limb accounted for 7.19%- 14.76% and 0.11%-6.36% of REM sleep stage, respectively. Two patients presented increasing of abnormal extremity movements during REM sleep stage. Conclusion AD patients may associate with RBD, which may be related to the injury of neural pathway engaged in REM regulation.
