摘要
目的:了解骨髓涂片示细胞增生低下的原因。方法:对66例骨髓涂片显示细胞增生低下(或极度低下)患者的同期同部位骨髓活检组织用塑料包埋半薄切片,做常规染色及网状蛋白染色,光镜下观察。结果:根据切片结果把引起骨髓涂片示细胞增生低下(或极度低下)的原因分为以下5类:细胞增生低下(10例,10/66);细胞增生旺盛(37例,37/66);网状纤维增加(6例,6/66);骨髓细胞增生旺盛伴网状纤维增加(9例,9/66);穿刺失败(4例,4/66)。结论:骨穿干抽或骨髓涂片示细胞增生低下(或极度低下)并非完全是骨髓细胞增生不良所致,骨髓涂片与活检相结合可提高对骨髓细胞增生程度的判断及血液病诊断的正确性。
Objective: To understand the reasons leading to the hypocellularity in bone marrow (BM)aspirate smear by observing the BM tissue pathology obtained by trephine biopsy. Methods: For 66 cases whose bone marrow showed hypocellularity in BM aspirate smear, trephine biopsy and reticulin staining were done at the same location in the same patient and at the same time as the BM aspirate smear was done. Results: The reasons leading to the hypocellularity in BM aspirate smear are due to: truly hypocellularity in 10 cases(10/66), hypercellularity in 37 cases(37/66), increased reticulin in 6 cases(6/66), hypercellularity with increased reticulin in 9 cases(9/66), failure of puncture in 4 cases(4/66). Conclusion: The hypocellularity in bone marrow smear is not caused totally by hypoplasia. Combining the two methods, bone marrow aspirate and trephine biopsy can increase the accuracy diagnosing the cellularity in BM and hematological diseases.
