摘要 目的:分析选择性冠状动脉造影(SCA)术中发生心室颤动(VF)的原因及其预防措施。方法:回顾性分析5050例SCA术中发生VF病例的资料。结果:SCA术中,7例发生VF(均为右冠造影时),其中桡动脉造影5例;用5F Terumo TIG造影导管4例;三支冠脉病变2例、双支冠脉病变1例;右冠脉发育细小3例、导管进入窦房结动脉1例、导管插入过深2例;5例患者存在不同程度造影剂滞留现象;5例患者发生VF前首先表现为冠脉内压力下降,2例无冠脉内压力下降。所有患者均1次电复律成功。结论:右冠细小、造影剂滞留、器材选择及操作不当是冠脉造影发生室颤的主要原因,充分认识并避免之可防止VF发生。 Objective: To study the cause and appropriate treatment of ventricular fibrillation during selective coronary angiography. Methods: Retrospectively analyzed the data of 5050 patients who were treated with selective coronary angiograph. Results: The seven of 5050 patients complicated with ventricular fibrillation when right coronary angiography was in progress. Five of seven patients were trans-radial artery coronary angiography, four patients using 5F Terumo TIG cather. In these seven patients, two was with three branches lesion, one was with double branch lesion, three with small right coronary artery (one with catheter into Sinus artery, two with deep insert), five of seven patients with contast media retention, five of seven patients with the pressure of coronary artery descending. All patents were successful cardioversion only with one electrical conversion. Conclusion: Small right coronary artery, contrast medium retention, and unfit instrument selection are contributed ventricular fibrillation during selective coronary angiography, but the complication can be avoid if be stressed.
