背景:近年来省时省力且能使根管形成良好锥度和形态的大锥度镍钛根管锉正逐步在临床推广,但与之相匹配的大锥度牙胶尖能否获得良好的根尖封闭效果尚未得到证实。目的:验证牙胶尖不同锥度对根尖区封闭性能的影响。设计、时间及地点:对比观察实验,于2008-12/2009-02在北京大学人民医院口腔科和中心实验室完成。材料:选择正畸拔除的上颌第一前磨牙,舍去扁平根管者,最后选出牙体完整,牙根发育完全,牙根长度近似双根管上颌第一前磨牙31颗。方法:离体上颌双根管第一双尖牙30颗,经预备后随机平均分成3组,0.02锥度组10颗,0.04锥度组10颗,0.06锥度组10颗,分别应用0.02锥度牙胶尖、0.04锥度牙胶尖和0.06锥度牙胶尖作主尖加Cortisomol根充糊剂侧方加压法进行根管充填,另外1颗为阳性对照组不进行根管充填。主要观察指标:用透明标本法配合染料渗透技术测量根尖染料的线性渗漏长度,评价各组根尖封闭能力。结果:阳性对照牙根管全长均有染料染色。0.02锥度组渗漏长度为(1.655±0.426)mm、0.04锥度组为(1.210±0.367)mm和0.06锥度组为(1.100±0.462)mm。0.04锥度组和0.06锥度组的根尖渗漏距离差异无显著性意义(P>0.05);0.02锥度组和0.04锥度组的根尖渗漏距离差异有显著性意义(P<0.05);0.02锥度组和0.06锥度组的根尖渗漏距离差异有显著性意义(P<0.05)。结论:MTWO预备根管后,冷牙胶侧方加压充填时大锥度牙胶尖能取得较好的根尖封闭。
BACKGROUND: A nickel-titanium instrument with big taper angle is gradually applied in clinic due to its excellent taper angle and appearance. However, it is poorly understood whether greater tapered gutta-percha cones has good effect on apical seal. OBJECTIVE: To verify the effect of gutta-percha cone with different tapered angles on apical area sealing. DESIGN, TIME AND SETTING: A comparison observation experiment was performed at the Department of Stomatology and Center Laboratory of People's Hospital of Peking University from December 2008 to February 2009. MATERIALS: Thirty-one extracted the first maxillary premolars teeth with completed root formation were collected. METHODS: Thirty teeth were randomly divided into 0.02 tapered angle (n=10), 0.04 tapered (n=10) angle 0.06 tapered angle (n=10) groups after MTWO preparation, which was filled with three different master cones and obturated with cold lateral condensation. The remained tooth was served as control group. MAIN OUTCOME MEASURES: The apical leakage was evaluated by the linear measurement combined with transparent specimen method, and the ability of apical seal was measured. RESULTS: The dye penetration of positive control group was along the whole canals. The length of penetration were (1.655±0.426) mm, (1.210±0.367) mm and (1.100±0.462) mm in 0.02 tapered angle, 0.04 tapered angle and 0.06 tapered angle groups, respectively. The apical sealing ability of 0.04 tapered angle group and 0.04 tapered angle group had no significant difference (P 〉 0.05). While the length of penetration had statistical differences between 0.02 tapered angle group and 0.06 tapered angle group (P 〈 0.05) or 0.02 tapered angle group and 0.06 tapered angle group (P 〈 0.05). CONCLUSION: Better apical seal can be obtained when using the big cones and obturated with cold lateral condensation after MTWO treatment.
