冻干治疗用母牛分枝杆菌菌苗配合全身化疗治疗耐多药肺结核 被引量:2

摘要 目的探讨冻干治疗用母牛分枝杆菌菌苗配合全身化疗治疗耐多药肺结核的效果。方法将84例耐多药肺结核患者分为观察组43例和对照组41例,所有患者均采用3ADVThZE/15DVThE全身化疗方案抗结核治疗,观察组在此基础上加用冻干治疗用母牛分支杆菌菌苗,前3个月每15 d肌肉注射1支(22.5μg),后15个月每月肌肉注射1支,共21次。比较2组的细胞免疫功能及痰菌阴转率、病灶显效率、空洞闭合率等方面的差异。结果2组治疗后9个月末及18个月末CD3+、CD4+、CD4+/CD8+均较治疗前升高(P<0.05,P<0.01),CD8+较治疗前下降(P<0.05,P<0.01),治疗后9个月末及18个月末CD3+、CD4+、CD4+/CD8+观察组明显高于对照组(P<0.01),CD8+2组比较差别无统计学意义(P>0.05);治疗9个月及18个月末观察组的痰菌阴转率、病灶显效率、空洞闭合率均高于对照组,差别有统计学意义(P<0.05,P<0.01)。结论冻干治疗用母牛分支杆菌菌苗能提高耐多药肺结核患者的细胞免疫功能,提高痰菌阴转率、病灶显效率及空洞闭合率,此方法值得在临床推广使用。 Objective To evaluate the efficacy of frozen-dried M. Vaccae Vaccine for therapy combined with general chemotherapy for multi-drug resistance pulmonary tuberculosis. Methods 84 cases with multi-drug resistance pulmonary tuberculosis were randomly divided into the observation group(43 cases) and control group (41 cases). All patients received the therapy of 3DVThAE/15DVThE. The patients in the observation group were injected Freezed-dried M. Vaccae Vaccine with 22. 5 μg once every 15 d in the first three months,once a month in the following fifteen months,21 times totally. Then comparisons were made between the two groups in the cellular immune function, the sputum tubercle bacillus conversion rate, focus improvement rate and cavity closing rate. Results By the end of the 9^th months and the 18^th months, the number of CD3^+, CD4 ^+,CD4^+/CD8^+ of all patients increased (P〈0.05,P 〈0.01 ) ,but the number of CD8^+ decreased(P 〈0.05,P 〈0.01 ) compared with pre-treatment in two groups. The number of CD3^+ , CD4^+, CD4 ^+/CD8^+ of the patients in observation group was higher than that in control group (P 〈0.01 ) ,but there was no difference in the number of CD8^+ between the two groups(P 〉 0. 05 ). The sputum negative conversion rate, focus improvement rate and cavity closing rate of the patients in the observation group were higher than those in the control group, there was significant difference between the two groups (P 〈 0. 05, P 〈 0.01 ). Conclusion Frozen-dried M. Vaccae Vaccine for therapy can improve the cellular immune function of the patients with multi-drugresistent pulmonary tuberculosis, can improve the sputum tubercle bacillus conversion rate, radiographic improvement rate and cavity closing rate. It is worthwhile to be used in clinic extensively.
