摘要
目的介绍改良中环指岛状皮瓣加髂骨植骨再造拇指Ⅲ°、Ⅳ°缺损的临床经验。方法对拇指Ⅲ°、Ⅳ°缺损的特殊病例采用将中环指岛状皮瓣设计于近中节，在近节指蹼间保留0．5-1cm皮肤，避免指蹼破坏，指血管神经束一并分离转位，供侧指神经与健侧指神经作端侧吻合，供区创面I期带真皮血管网植皮，中环指岛状皮瓣转移后瓦合加髂骨植骨再造拇指。结果临床应用共7例，中环指皮瓣均顺利存活。经6月-1年随访，再造指外观满意，两点辨别觉1-2mm，对掌对指功能恢复良好。结论改良中环指岛状皮瓣加髂骨植骨再造拇指是一种简单有效可靠的方法。
Objective To introduce the clinical experiences of reconstruction of Ⅲ and Ⅳ degree defect of the thumb with modified island flaps of middle and ring fingers combined with crest bone implant.Methods Island flaps of middle and ring fingers were designed at the proximal part of the middle phalanx, 0.5-1cm skin adjacent to the middle phalanx remained to protect the finger web,and the digital neurovascular bundle was dissected and transfered, between the donor and the recipient area. The donor area wound was the nerve was sutured grafted with the subderrnal vascular network skin, island flaps of middle and ring fingers were transferred, then combined with crest bone implant to reconstruct thumb.Results 7 cases were applied clinically and all survived completely. With a follow-up of 6-12 months, the Contour of the thumb after reconstruction was satisfactory, the two-point of static discrimination was 1-2mm and the recovery of the function as thumb opposition was good.Conclusion Thumb reconstruction with modified island flaps of middle and ring fingers combined with crest bone implant was a simple and effective and reliable operative method.
