摘要
目的:为提高AO微型钢板螺钉治疗掌骨骨折效果。方法:笔者总结了2006~2008年用微型钢板螺钉治疗30例掌骨骨折的临床资料。共19例患者,30处骨折,闭合性骨折16处,开放性骨折14处。骨干部骨折用4孔AO微型钢板固定,干骺部骨折用T或L形AO微型钢板固定;大斜形或螺旋形、关节内骨折用螺钉加压固定。术后即行主被动活动。结果:术后22处关节活动达到优秀,其中,开放性骨折8处,闭合性骨折14处。8处发生了并发症,其中,开放性骨折6处,闭合性骨折2处。结论:AO微型钢板及螺钉为掌骨骨折提供了牢稳的固定,有利于早期功能康复训练,获得了较好的手功能恢复。
Objective: To improve the therapeutic effect of miniplate and screw in the treatment of metacarpal fractures. Methods: Data of 30 cases of fractures treated with miniplate from 2006 to 2008 were reviewed.Among 30 cases of patients, there were 16 cases of closed fractures,14 cases of open fractures, the 4-hole 2.0 mm AO miniptate was used for midshaft fracture of the metacarpals ;the "T" or "L"AO shaped plate was used for urmtabie intraartieular and periarticular fractures;the screw was used for long ob lique diaphyseal fractures,spiral fracture and some of intraarticular fraeturs. Immediate active and passive motion was done postoperatively.Results: The quality of recovery of joint motion was assessed by the Total Active Flexion Scale,22 cases were excellent, including 8 cases of open and 14 cases of closed fractures. Complications occurred in 8 cases (6 cases of open and 2 cases of closed fractures) among 30 eases of fractures. Conclusion: AO miniplate and screws can provide the stable fixation for metacarpal fractures,and stabilize the fracture site allowing immediate motion postoperatively,so that the hand function recovers better and more quickly.
