摘要
目的：对寰枢椎后路融合术治疗创伤性寰枢椎不稳进行回顾总结。方法：对３４例创伤性寰枢椎不稳患者进行寰枢椎后路融合术，包括齿状突骨折２９例，横韧带断裂５例。所有患者行寰枢椎钢丝固定自体髂骨移植术，其中Ｇａｌｉｅ法固定２１例，Ｂｒｏｏｋｓ法固定１３例。结果：术后进行颈椎侧位Ｘ线片随访３～６３个月不等（平均９个月），３４例均获骨性愈合。结论：寰枢椎后路融合术适用于陈旧性不稳定齿突骨折、伴有不愈合倾向齿突骨折及横韧带断裂患者，解剖复位。
Objective:To review and summarize retrospectively an experience in posterior atlantoaxial fusion for the treatment of traumatic atlantoaxial instability. Methods:Thirtyfour patients with traumatic atlantoaxial instability including 29 patients with unstable odontoid fractures and 5 with a disrupted transverse ligament, were treated surgically with posterior atlantoaxial fusion. All patients were operated by atlantoaxial arthrodesis using wire fixation with autologous bone grafts. Gallies fixation was performed in 21 patients and Brooks fixation was used in 13 patients. Results:The patients were followed up postoperatively with lateral cervical radiographs for average of 9 months (range 3～63 months). All patients had solid osseous unions. Conclusion:Posterior atlantoaxial fusion is recommended for old unstable odontoid fractures, Type Ⅱ odontoid fractures with factors developing nonunion and transverse ligament disruption.It is emphasized that anatomic reduction and adequately controlling atlantoaxial motion are the important measures for successful operation.
