2型糖尿病无症状低血糖患者膳食结构分析 被引量:11

摘要 目的了解2型糖尿病患者无症状低血糖与膳食结构的关系。方法对46例2型糖尿病患者采用动态血糖监测系统,筛查出无症状低血糖患者63例次(无症状低血糖组),与63例次患者(非低血糖组)前1d或后1d相应时间段未发生低血糖时三大产能营养素实际摄入量进行分析比较,分析比较使用营养计算器V1.6。结果无症状低血糖63例次,发生在晚夜间31例次,占49%;年龄>60岁者39例次,占62%;非低血糖组碳水化合物的摄入量高于无症状低血糖组(P<0.01),非低血糖组脂肪摄入量低于无症状低血糖组(P<0.01)。2型糖尿病患者3大营养素产能百分比均存在不合理现象,尤其是无症状低血糖组患者存在脂肪产能比过高、碳水化合物产能比严重偏低现象。结论2型糖尿病患者膳食结构不合理是导致无症状低血糖的主要原因;当碳水化合物产能比例低、脂肪产能比例高时,有发生无症状低血糖的危险;血糖控制理想时警惕低血糖,"分餐制"有利于预防无症状低血糖的发生;老年糖尿病患者对糖尿病膳食理解不透彻,更应警惕无症状低血糖的发生。医护人员应提高对无症状低血糖的认识,合理用药,同时加强糖尿病膳食结构教育,预防无症状低血糖的发生。 Objective To explore the relationship between the asymptomatic hypoglycemia among Type 2 diabetic patients and their dietary structure. Methods By means of dynamic blood glucose monitoring system, 63 case times with asymptomatic hypoglycemia (AH group) were screened out of 46 diabetic patients. And then a comparison was made, by using V1.6, of the actual intake of three major nutrients of the AH group and that of the non-hypoglycemia group (NH group, 63 case times) in the corresponding period of time one day before or after. Results Of the 63 case times of AH, 31 occurred at night, accounting for 49%, and 39 with those aged above 60, accounting for 62%. The intake of carbohydrates by AH group was smaller then that of NH group (P〈0.01), while the intake of fat of the former, greater than that of the latter (P〈0.01). There existed irrationalities with the structure of the three major nutrients among diabetic patients, especially the over-lntake of fat and under-intake of carbohydrates among AH group. Conclusion Irrational dieting is the major cause of asymptomatic hypoglycemia. Low proportion of carbohydrates and high proportion of fat involve the risk of asymptomatic hypoglycemia. In case of good blood glucose control, guard should be taken against hypoglycemia; the practice of one meal being served for times is conducive to preventing this. Senior diabetic patients, who lack a thorough understanding the dieting, should be more watched out for asymptomatic hypoglycemia, and medical personnel should heighten their understanding of asymptomatic hypoglycemia, administer drugs in a reasonable way, and conduct education on dieting for diabetic patients more rigorously.
