摘要
目的探讨丙泊酚不同效应室浓度对人瞬态诱发耳声发射(TEOAE)的影响,以探索TEOAE信号与麻醉深度的相关性。方法选择全身麻醉下行择期手术、ASAⅠ或Ⅱ级、年龄18~49岁听力正常的患者15例,麻醉诱导期丙泊酚靶控输注效应室浓度依次设定为1、2、3和4μg/mL,不同效应室浓度代表不同麻醉深度。Smart OAE耳声发射检测仪记录诱导前及诱导过程中不同麻醉深度总反应幅值、总重复率及各频带(1.0、1.5、2.0、3.0和4.0kHz)相应TEOAE的反应幅值和信噪比。采用自身对照,研究不同麻醉深度下各频带TEOAE相关参数的变化。结果诱导前与诱导过程中不同麻醉深度总反应幅值、总重复率及各频带TEOAE幅值反应稳定,但不同麻醉深度总反应幅值、总重复率及各测试频带反应幅值及信噪比的变化差异没有显著性(P>0.05)。结论丙泊酚不同效应室浓度对人TEOAE总反应幅值、总重复率及各测试频率带相应的反应幅值、信噪比的影响不明显;丙泊酚不同效应室浓度下TEOAE信号尚不能反应麻醉深度。
[Objective] To study the effects of Propofol under different effect-site concentration on signals of transient evoked otoacoustice emissions (TEOAE) of adult and to find an approach to measure depth of anaesthesia. [Methods] Fifteen ASA I or II patients aged 18-49 years undergoing operation below cervical part under general anaesthesia were enrolled in this study. Patients with aural disease were excluded and no patients were taking any drugs that might influence TEOAE signals. Anesthesia was maintained with Propofol by TCI (target-controlled infusion), the effect-site concentration was set at 1, 2, 3 and 4 μg/mL respectively and TEOAE signals was monitored and recorded response amplitude, wave reproducibility, Band (1, 1.5, 2, 3 and 4 KHz) response amplitude, Band Signal to Noise Ratio (SNR) by SmartOAE (3.X, HIS, USA) before and after anesthesia, then the data were analysed with repeated measurements using SPSS 13.0 statistical software. [Results] It was not significant for response amplitude, wave reproducibility, Band response amplitude, Band SNR under different effect-site concentration of Propefol (P 〉0.05). [Conclusion] Effects of Propofol under different effect-site concentration on TEOAE signals of adult is not obvious and TEOAE signals do not reflect depth of anaesthesia.
