摘要 目的了解创伤患者下呼吸道感染的病原菌分布及耐药特点,为院内感染控制和合理用药提供参考。方法收集2007年1月至2008年6月创伤患者下呼吸道分泌物中被分离的1 126株目标致病菌进行分类和药敏试验。结果革兰阳性菌201株,占17.8%,以金黄色葡萄球菌(14.4%)为主;革兰阴性菌870株,占77.3%,位列前3位的是绿脓假单胞菌(26.6%)、醋酸钙鲍曼复合杆菌(20%)和肺炎克雷伯氏菌(11.6%);真菌55株,占4.9%,以白色念株菌为主。药物敏感试验结果显示,革兰阳性菌对万古霉素、替考拉宁等抗生素的耐药率较低,革兰阴性菌对头孢吡肟、哌拉西/他唑巴坦和亚胺培南等抗生素的耐药率相对较低。结论创伤患者下吸道感染致病菌以革兰阴性杆菌为主,且耐药率较高,应加强细菌耐药性监测及抗菌药物管理,以指导临床合理用药。 Objective To investigate the distribution and antibiotic resistance of pathogenic bac- teria from lower respiratory infection of trauma patients, and to provide scientific reference for control of hospital onset of infection and rational administration clinically. Methods Totally 1 126 strains of pathogenic bacteria were isolated from the secretions in lower respiratory tract of trauma patients dur- ing January 2007 to June 2008 and classified, given drug sensitivity test. Results Totally 201 strains (17.8 %) were gram positive bacteria, mainly Staphylococcus aureus (14.4 %) ; 870 strains (77.3 %) were gram-negative bacteria, mainly including Pseudomonas aeruginosa (26.6%), Acinetobaeter bau- mannii (20%), and Klebsiella (11.6%)%; and 55 strains (4.9%) were fungi, mainly Candida albi- cans. Drug sensitivity test results displayed that vancomycin and teicoplanin was the most sensitive an- tibiotics for treating gram-positive coccus infection. As gram negative bacteria as concerned, the re- sistant rate was relative low to eefepime, piperacillin/tazobaetam, and imipenem. Conclusion Gram- negative bacteria are the leading pathogens in lower respiratory tract infection of trauma patients. The drug resistance rate is higher. It is urgent to strengthen monitoring of bacterial resistance and manage- ment of antibacterials for rational use of antibiotics.
